Whole-body energy metabolism and metabolic capacity of skeletal muscules in malnourished patients before and after total parenteral nutrition

A study evaluated whether a 2-week treatment by total parenteral nutrition (TPN) to induce positive energy and nitrogen balance also would induce an improved metabolic capacity in skeletal muscles of 18 depleted cancer patients (mean age, 64) and 7 malnourished, non-cancer patients (mean age, 62). TPN treatment included glucose, lipid, and amino acids (200 kcal/g nitrogen), where 30% of the non-protein energy was due to glucose and 70% was due to lipid. TPN induced an increase in body weight and body potassium; positive whole-body energy and nitrogen balance were achieved and associated with an improved translational capacity for skeletal muscle protein synthesis (in vitro). The finding that muscle RNA was increased after TPN indicated that in vivo transcriptional events were involved. (wz)
