Utilization of D-methionine during total parenteral nutrition in postsurgical patients

Utilization of intravenously administered D-methionine was measured in morbidly obese subjects fed parenterally after elective gastric bypass surgery. 5 patients were infused with DL methionine and 4 with L methionine. In the DL group 90-98% of the excreted methionine was the D type. The data indicates D-methionine is poorly utilized in postsurgical patients receiving total parenteral nutrition when given as DL-methionine in the presence of other amino acids and glucose. Present data demonstrate the difficulty in extrapolating animal data to humans. Although a number of animal species efficiently utilize orally or intravenously fed D-methionine, postsurgical adults and normal infant humans do not.
