A literature review summarizes the known effects of alcohol on metabolic homeostasis in the diabetic patient and, with respect to the current state of knowledge, suggests what practical advice such be given to diabetic patients. Attention is given to the gastrointestinal absorption of alcohol, its caloric content, and its effects on lipid metabolism and its short and longer term effects on carbohydrate metabolism. The effects of alcohol on diabetic complications, the interactions between oral hypoglycemic drugs (sulphonylureas, biguanides) and alcohol, what level of alcohol intake is acceptable, and the effect of alcohol on driving safety, also are discussed. Thirteen recommendations for diabetic patients concerning alcohol consumption are included.(wz)
