Der Einfluss von bone morphogenetic protein endothelial cell precursor derived regulator (Bmper) und bone morphogenetic protein 4 (Bmp4) auf die Entstehung von thorakalen Aortenaneurysmen im Mausmodell | The influence of Bmper and Bmp4 on the development of thoracic aortic aneurysms in mice

Bone morphogenetic proteins (BMPs) zählen zur transforming growth-factor-β (TGF-β)-Superfamilie und sind für die Regulation zahlreicher Signaltransduktionen, auch außerhalb von Knochen- und Knorpelgewebe, beteiligt. So ist bekannt, dass BMPs Einfluss auf die frühe Herzentwicklung nehmen und neben diesen auch die biologische Funktionalität und Homöostase der Blutgefäße beeinflussen. Bislang sind zwei hereditäre vaskuläre Erkrankungen direkt mit dem BMP-Signalweg assoziiert: hierzu zählen die pulmonal arterielle Hypertonie und die hereditäre Teleangiektasie. Die pleioptropen und zelltypspezifischen Effekte der BMPs auf Endothelzellen und vaskuläre glatte Muskelzellen lassen weitere BMP-assoziierte kardiovaskuläre Erkrankungen vermuten. In vitro Vorversuche zeigten, dass BMP4 und der BMP-Modulator BMPER (BMP endothelial cell precursor derived regulator) den kontraktilen Phänotyp von pulmonal-arteriellen glatten Muskelzellen (engl. PASMCs) fördert und ein Fehlen zu einer erhöhten Proliferation und Migration, sowie in Synthese von Matrixmetalloproteinasen (MMPs) resultiert (Baur 2015, Prölß 2018). Dies führte zur Annahme, dass eine Dysregulation von BMP4 und BMPER durch biochemische und mechanische Reize mit strukturellen und funktionellen Veränderungen im Aortengewebe einhergeht und so zur Pathogenese von thorakalen Aortenaneurysmen beitragen kann. Aortenaneurysmen sind eine tödliche Erkrankung, für die es zum jetzigen Zeitpunkt keine medikamentöse Therapie gibt, die die Progression nachweislich reduziert. Umso wichtiger ist die Erforschung der beteiligten Signalwege, die zu einer meist multifaktoriellen Pathogenese führen. Für diese in vivo Studie wurde Mäusen für 28 Tage eine osmotische Minipumpe mit Angiotensin-II (AngII) in den Nacken implantiert und durch die Entstehung von Bluthochdruck zur Induktion von Aortenaneurysmen genutzt. Im ersten Versuchsabschnitt wurde mit der Mauslinie C57/BL6N–Bmper gearbeitet, bei der die Auswirkungen einer globalen heterozygoten Defizienz von Bmper auf die Pathomechanismen in der Gefäßwand und somit der Entstehung von thorakalen Aortenaneurysmen geklärt werden sollten. In der Echokardiografie zeigte sich bei den mit AngII behandelten Mäuse eine Dilatation des Aortenbogens, jedoch keine Veränderung des Durchmessers der Aorta durch die heterozygote Defizienz von Bmper. Histologische und molekularbiologische Daten zeigen einen Verlust von kontraktilen vSMC-Markern, wie beispielsweise α-Sma und Myh11, durch die heterozygote Defizienz von Bmper. Zusätzlich kam es zu einer Destruktion von elastischen Fasern, zu einer erhöhten Steifigkeit durch die Einlagerung von Kollagen in die Gefäßwand und der erhöhten Expression der typischen Marker Mmp2 und 9. Zusammengefasst resultieren diese Veränderungen in einem Verlust der Elastizität und Kontraktilität sowie einer Zunahme der Wandsteifigkeit, die eine Progression von Aortenaneurysmen im thorakalen Abschnitt begünstigen können. In der zweiten hier durchgeführten Studie wurde die induzierte Defizienz von Bmp4 in vSMCs und die daraus resultierenden Veränderungen auf die Integrität und Funktionalität des Aortengewebes untersucht. So konnte in der vorliegenden Arbeit zwar keine deutliche Reduktion der kontraktilen Marker auf mRNA- und Proteinebene gezeigt werden, jedoch lassen immunhistologische Färbungen aus einer früheren Arbeit und eine erhöhte Apoptoserate auf die Veränderung in der vSMCs Popolation schließen (Prölß 2018). Im Aortengewebe der vSMCΔBMP4 Mauslinie konnte zudem eine erhöhte Expression von Mmp2 und Mmp9 gezeigt werden, welches mit den Daten aus den histologischen Untersuchen auf den Wechsel vom kontraktilen vSMC-Typ in einen synthetisierenden Phänotyp schließen lässt. Anhand dieser und der zuvor erhobenen Daten (Prölß 2018) in diesem Projekt lässt sich ein protektiver Effekt von Bmp4 auf den Phänotyp der glatten Muskelzellen zeigen und ein protektiver Effekt für die Genese von thorakalen Aortenaneurysmen vermuten. Somit bildet diese Arbeit eine wichtige Grundlage für das Verständnis des BMP-Signalwegs in Bezug auf die Entstehung von thorakalen Aortenaneurysmen und der zugrundeliegenden Veränderungen der vaskulären glatten Muskelzellen. Auch für andere vaskuläre Erkrankungen, in denen die Veränderungen von vSMCs maßgeblich zur Genese der Pathologien beitragen, können diese Ergebnisse von hohem Interesse sein.

Bone morphogenetic proteins (BMPs) belong to the transforming growth-factor-β (TGF-β) superfamily and are involved in the regulation of numerous metabolic processes besides bone and cartilage tissue. It is known that BMPs have an influence on early cardiac development and, in addition to these, also affect the biological functionality and homeostasis of blood vessels. To date, two hereditary vascular diseases are directly associated with the BMP signaling pathway: pulmonary arterial hypertension and hereditary telangiectasia. The pleioptrophic and cell type-specific effects of BMPs on, e.g. endothelial cells and vascular smooth muscle cells suggest further BMP-associated cardiovascular diseases. In vitro preliminary experiments showed that BMP4 and the BMP-modulator BMPER (BMP endothelial cell precursor derived regulator) promote the contractile phenotype of pulmonary arterial smooth muscle cells (PASMCs) and that a deficiency leads to increased proliferation and migration, as well as to the synthesis of matrix metalloproteinases (MMPs) (Baur 2015, Prölß 2018). Thus, the hypothesis is that dysregulation of BMP4 and BMPER by biochemical and mechanical stimuli is associated with structural and functional changes in aortic tissue and may contribute to the pathogenesis of thoracic aortic aneurysms. Aortic aneurysms are a fatal disease for which there is currently no drug therapy that has been shown to reduce progression. Therefore it is of upmost importance to investigate the involved signaling pathways such as the BMPs. For this in vivo study, an osmotic mini-pump with Angiotensin-II (AngII) was implanted into the neck of mice for 28 days to induce aortic aneurysms via hypertension. In the first part of the study the mouse line C57/BL6N-Bmper was used to elucidate the effects of a global heterozygous deficiency of Bmper on pathomechanisms in the vessel wall and along this line the development of thoracic aortic aneurysms. Echocardiography showed a dilatation of the aortic arch in AngII treated mice, but no change in aortic diameter due to the heterozygous deficiency of Bmper. Histological and molecular biological data indicated the loss of contractile vSMC markers due to the heterozygous deficiency of Bmper. In addition, there was destruction of elastic fibers, increased stiffness due to the deposition of collagen in the vessel wall and an increased expression of Mmp2 and 9 that are typical markers for enhanced extracellular matrix remodeling. In summary, these changes result in a loss of elasticity and contractility as well as an increase in wall stiffness, which may favor a progression of aortic dilatation and dissection in the thoracic segment. The second part of the study examined the induced deficiency of Bmp4 in vSMCs and the resulting changes in aortic tissue integrity and functionality. Although the present study could not show a significant reduction of contractile markers at the mRNA and protein level, immunhistological staining from a previous study and an increased rate of apoptosis suggest that the vSMCs are altered. In addition, an increased expression of Mmp2 and Mmp9 was shown in the aortic tissue of the vSMCΔBMP4 mouse line, which, together with the data from the histological examinations, suggests a change from the contractile vSMC type to a synthesizing phenotype. Based on these and the previously collected data, a protective effect of Bmp4 on the contractile vSMC phenotype is shown and, therefore, a protective effect against the genesis of thoracic aortic aneurysms can be assumed. Thus, the current thesis provides an important basis for understanding the BMP-signaling pathway in relation to the development of thoracic aortic aneurysms and the underlying changes in vascular smooth muscle cells. These results may also be of great interest for other vascular diseases, in which the changes in vSMCs play a major role in the pathogenesis of diseases.
