Der Einfluss des Proteasominhibitors Bortezomib auf die kardiale Hypertrophie in der Dahl Salz-sensitiven Ratte | The Effect of the Proteasome Inhibitor Bortezomib (Velcade®) on the Cardiac Hypertrophy in Dahl Salt-Sensitive Rats

Hintergrund: Die kardiale Hypertrophie stellt einen Risikofaktor für diverse kardiovaskuläre Komplikationen, wie z. B. Herzinsuffizienz und Schlaganfall dar. Die derzeitigen therapeutischen Möglichkeiten sind jedoch begrenzt, so dass ein großer Handlungsbedarf für die Entwicklung neuer Therapieverfahren besteht. Das Ubiquitin- Proteasom-System (UPS) stellt den wichtigsten nicht- lysosomalen Abbauweg intrazellulärer Proteine in eukaryotischen Zellen dar und spielt bei der Kontrolle von zahlreichen zellulären Prozessen eine wichtige Rolle. Eine Hemmung des Proteasoms supprimierte in vitro die Hypertrophie von neonatalen Rattenkardiomyozyten (nRC). Ziel: In dieser Arbeit wurde untersucht, ob der niedrig-dosierte Proteasominhibitor Bortezomib anti- hypertrophe Effekte in dem In-vitro-Modell der neonatalen Rattenkardiomyozyten und dem Tiermodell der Dahl Salz-sensitiven Ratte (DSSR) ausübt. Methoden und Ergebnisse: Die In-vitro-Untersuchungen in nRC ergaben, dass Bortezomib die Chymotrypsin-ähnliche Aktivität des Proteasoms inhibierte. Dies war mit einer deutlichen Supprimierung des Isoproterenol-induzierten hypertrophen Wachstums von nRC assoziiert. So konnten sowohl eine Reduzierung der Zellfläche, als auch verringerte mRNA Expressionen der Hypertrophie-Markergene g-MHC und ANP beobachtet werden. Währenddessen konnte kein Einfluss auf die Zellviabilität ausgemacht werden.In dem zweiten Teil dieser Arbeit wurde DSSR entweder der niedrig-dosierte Proteasominhibitor Bortezomib (50 μg/kg KG) oder das Placebo NaCl injiziert. Die Behandlung erfolgte zweimal wöchentlich über acht (1. Versuch) bzw. siebeneinhalb Wochen (2. Versuch). Dabei konnten mäßige anti- hypertrophe Effekte von Bortezomib in der Druck-induzierten kardialen Hypertrophie festgestellt werden. Die Kardiomyozyten- Querschnittsfläche und der Kollagenanteil in dem linken Ventrikel waren tendenziell verringert. Obwohl diese Effekte nicht signifikant waren, wurde das absolute und relative Herzgewicht (HG/TL) (tendenzielle bzw. signifikante Unterschiede in dem ersten bzw. zweiten Versuch) reduziert. Ein Einfluss auf die Hypertrophie- bzw. Fibrose-Markergene く-MHC und Col III (a1) konnte nicht beobachtet werden. Untersuchungen in dem vaskulären System der DSSR ergaben, dass der niedrig- dosierte Proteasominhibitor anti-hypertensiv wirkte. Der arterielle Blutdruck wurde tendenziell bzw. signifikant reduziert (1. bzw. 2. Versuch). Dies war mit einer im Trend verbesserten Vasoreaktivität in Antwort auf den Vasodilatator Acetylcholin bzw. den Vasokonstriktor ET-1 assoziiert. Weitere Analysen ergaben, dass Bortezomib die mRNA Expression des inflammatorischen Markers VCAM-1 signifikant reduzierte. Während die Expression von ET-1 tendenziell reduziert wurde, konnte kein Einfluss des Inhibitors auf die eNOS Expression beobachtet werden. Außerdem konnten hämatologische und hämatopoetische Nebenwirkungen des Proteasominhibitors ausgeschlossen werden. Schlussfolgerung: Der niedrig-dosierte Proteasominhibitor Bortezomib hatte eine antihypertrophe Wirkung auf kultivierte nRC. In hypertensiven DSSR vermittelte der niedrigdosierte Proteasominhibitor kardiovaskulär protektive Effekte, die in ihrer Summe zu einer verminderten Morbidität und Mortalität und zu einer reduzierten kardialen Hypertrophie führten.

Background: The cardiac hypertrophy is a risk factor for cardiovascular complications, such as heart failure and stroke. The present therapeutic possibilities are limited, so there is urgent need for the development of new therapy approaches. The ubiquitin-proteasomesystem (UPS) is the major nonlysosomal degradation pathway of intracellular proteins in eukaryotic cells. It plays an important role in the control of numerous cellular processes. Inhibition of the proteasome suppressed the hypertrophy in vitro of neonatal rat cardiomyocytes (nRC). Purpose: In this study it was investigated whether the low-dose proteasome inhibitor bortezomib has anti-hypertrophic effects in the in-vitro-model of the neonatal rat cardiomyocytes as well as in the animal model of Dahl salt-sensitive rats (DSSR). Methods and Results: The in-vitro-experiments in nRC demonstrated that bortezomib inhibits the chymotrypsin-like activity of the proteasome. This was associated with a pronounced suppression of isoproterenol-induced hypertrophic growth from nRC. Furthermore, a reduction of the cell surface, as well as decreased mRNA expression of the hypertrophic marker genes g-MHC and ANP was observed in cardiomyocytes. However, no influence on the cell viability was monitored. In the second part of this study DSSR were injected with a low-dose of proteasome inhibitor bortezomib (50 μg/kg body weight) or with saline as a placebo. The treatment occurred twice weekly over a period of eight weeks (1st set up) or 7.5 weeks (2st set up). Here antihypertrophic effects were determined through the pressure-induced cardiac hypertrophy by bortezomib. The cardiomyocyte cross-sectional area and the percent volume fraction of collagen in the left ventricle were slightly reduced. Although these effects were not significant, the absolute and relative heart weight (HW/TL) (slightly or significant differences in the 1st or 2st set up) were reduced. An influence on the hypertrophic or fibrosis marker genes く-MHC and Col III (a1) was not observed. Analysis of the vascular system from DSSR showed that low-dosed proteasome inhibition was anti-hypertensive. The arterial blood pressure was slightly or significant reduced (1st or 2st set up). This was associated with an improved vasoreactivity. Further analysis demonstrated that bortezomib reduced the mRNA expression of the inflammatory marker VCAM-1 significantly. On the other hand expression of ET-1 was slightly reduced and eNOS expression was not altered up on proteasome inhibition. In addition, hematological and hematopoietic side effects of the proteasome inhibition were nonexistent. Conclusion: Low-dosed proteasome inhibitor bortezomib had an anti-hypertrophic effect on cultured nRC. In hypertensive DSSR the low-dosed proteasome inhibitor mediated cardiovascular protective effects resulted in a reduced morbidity and mortality as well as decreased cardiac hypertrophy
