Klinische und radiologische Verlaufsuntersuchung an den Strahlbeinen der Vorderextremitäten von Pferden | ein Hinweis auf Entwicklung und klinische Relevanz von Befunden in Bezug auf den Röntgenleitfaden 2007 (RöLF 07) | Clinical and radiological course investigation in the sesamoid bones of the forelimbs of horses | a study of the development and clinical relevance of findings in relation on the radiology manual in 2007

In der vorliegenden Arbeit sollte im Rahmen einer klinischen und radiologischen Verlaufsuntersuchung der Strahlbeine der Vorderextremitäten von Auktionsanwärtern des Westfälischen Pferdestammbuches e.V. in Anlehnung an den Röntgenleitfaden (2007) die Entwicklung und klinische Relevanz der erhobenen Befunde untersucht werden. In der Literaturübersicht werden zunächst die Entstehung des Röntgenleitfadens und die ihm zugrundeliegenden Prinzipien dargestellt, sowie ein kurzer Überblick über die Röntgenanatomie von Zehe und Strahlbein gegeben. Im Weiteren wird die klinische und radiologische Untersuchung im Rahmen der Kaufuntersuchung des Pferdes beschrieben und das Auftreten und die Bedeutung klinischer und röntgenologischer Veränderungen erläutert. Die Studie wurde an 140 Pferden der Zuchtgebiete Westfalen und Rheinland in einem Abstand von ½ bis zu 6 Jahre nach der Erstuntersuchung durchgeführt. Für die Auswertung der Erstuntersuchung stand archiviertes Daten- und Bildmaterial sowohl der klinischen, als auch radiologischen Untersuchung zur Verfügung. Im Rahmen der Verlaufsuntersuchung konnten alle Pferde radiologisch, aber nur ein Teil klinisch untersucht werden. Die Daten der klinischen Untersuchung wurden vergleichend ausgewertet, das Röntgenbildmaterial anhand des Befunderhebungsschlüssels beurteilt und die Pferde dementsprechend klassifiziert. Dabei wurde ein besonderes Augenmerk auf die Entwicklung der Strahlbeinbefunde sowie der Klassifizierungen innerhalb des Untersuchungszeitraumes gelegt, unter Berücksichtigung klinischer Erscheinungen und der im Röntgenleitfaden gegebenen prognostischen Einschätzungen. In der Schlussfolgerung werden die im Rahmen der Untersuchung erhobenen Aussagen hinsichtlich der Entwicklung radiologischer Befunde und ihrer klinischen Relevanz aufgeführt. So konnte festgestellt werden, dass der überwiegende Teil der untersuchten Pferde innerhalb des Untersuchungszeitraumes in der eingangs befundeten Röntgenklasse verbleibt. Radiologische Befunde am Strahlbein verändern sich, aber in der Regel nur langsam und ohne bedeutende Verschlechterungen. Der hohe Anteil klinisch lahmfreier Pferde sollte bewusst machen, dass nicht alleinig die Röntgenuntersuchung und Klassifizierung die Kaufentscheidung eines Pferdes prägen sollte. Ein hoher Anteil von Pferden mit Befunden der Klasse III-IV wurde über Jahre erfolgreich ohne klinische Erscheinungen und Befundverschlechterungen im Sport eingesetzt. Des Weiteren verlangen die vielen verschiedenen Faktoren, die die Röntgenbildqualität beeinflussen eine kritische Einstellung gegenüber der Röntgendiagnose und bei unklaren Befunden sollten ergänzende Aufnahmen empfohlen werden. Die im Röntgenleitfaden gegebenen prognostischen Aussagen zur klinischen Relevanz von Befunden lassen sich im Rahmen dieser Studie übernehmen. Aufgrund der geringen Zahl an klinisch auffälligen Probanden und einer nicht durchgeführten Diagnosestellung kann jedoch keine allgemein gültige Aussage getroffen werden. Dies könnte eine weitere Studie mit einer größeren Probandenzahl und einhergehender Diagnosestellung bei klinischen und radiologischen Verdachtsbefunden leisten.

In the present study the development and clinical relevance of the upraised findings is examined within the scope of a clinical and radiological course investigation of the sesamoid bones of the forelimbs of auction candidates of the “Westfälisches Pferdestammbuch e.V.” according to the radiology manual in 2007. In the literature overview first the origin of the x-ray guide is shown and the principles forming the basis of it, as well as a short overview of the radiological anatomy of toes and sesamoid bones. Subsequently the clinical and radiological examination is described within the scope of the purchase in investigation of the horse and the appearance and the relevance of clinical and x-ray changes are explained. The study was carried out with 140 horses of the breeding areas Westphalia and Rhineland rainging in age from six months to 6 years according to the first investigation. Archive data material the clinical, as well as the radiological examinations was available for the evaluation of the first examination. Within the scope of the course investigation all horses could be examined radiologically, but only one part was examined clinically. The data of the clinical investigation were evaluated comparatively, the x-ray pictures were judged with the help of the findings elevation key and the horses were classified accordingly. Besides, special attention was laid to the development of the sesamoid bones findings as well as the classification within the investigation period, taking into account clinical phenomena and the prognostic appraisals given in the radiology manual. In the conclusion the statements upraised within the scope of the investigation are issued concerning the development of radiological findings and their clinical relevance. Thus it could be found out that the majority of the examined horses within the investigation period remained in the x-ray classification started at the beginning. Radiological findings in the sesamoid bones change, but as a rule do so only slowly and without significant deteriorations. The high percentage of clinically lame-free horses should indicate that the x-ray examination and classification should not be the sole basis of a purchase decision of a horse. A high percentage of horses with findings of the class III-IV were successfully used for years without clinical phenomena and findings at deteriorations in sports. Furthermore many different factors indicate that the x-ray image quality influences the x-ray diagnosis to a critical degree and with unclear findings complementary show methods of examination are recommended. The prognostic statements given in the radiology manual to the clinical relevance of findings can be taken over within the scope of this study. Nevertheless, on account of the low number of clinically relevant test animals and the lack of diagnosis position no generally-valid statement can be issued. Another study could achieve this with a bigger test sample and following diagnosis positions with clinical and radiological suspected findings.
