Correlações entre ACTH endógeno e relação altura da hipófise / área cerebral em cães com hipercortisolismo pituitário-dependente

Hipercortisolismo pituitário-dependente (HPD) é uma endocrinopatia que acomete frequentemente cães. É causada pela presença de um corticotropinoma, que tem por característica a secreção desregulada do hormônio adrenocorticotrófico (ACTH). Para a caracterização do tumor, a realização de exames de imagem do crânio é indicada. Contudo, o alto custo e os risco inerentes ao exame acabam limitando sua realização. Dessa forma, é importante a busca por biomarcadores de crescimento dos corticotropinomas que reduzam a necessidade de exames de imagem. O objetivo deste estudo é avaliar a concentração de ACTH ([ACTH]) e sua relação com tamanho hipofisário em cães com HPD ao diagnóstico e após tratamento com trilostano em monoterapia ou associado à cabergolina, e a relação do Escore Clínico da Síndrome de Cushing (ECSC) com estas duas variáveis. Os dados apresentados incluem pacientes atendidos no HCV-UFRGS, VetUnity-SP e na Universidade de Bologna -Itália provenientes de um estudo multicêntrico avaliando impacto da adição de cabergolina ao tratamento do HPD com trilostano. Foram selecionados 38 pacientes recém-diagnosticados (T0), os quais foram divididos nos grupos Tri (Grupo Tri, n = 18) e Cab+Tri (Grupo Cab + Tri, n =20) e monitorados por um ano (T360). A mensuração da [ACTH] respeitou cuidados pré-analíticos, sendo mensurada por quimioluminescência (Immulite 1000), e a relação altura hipófise/área cerebral (razão P/B) foi definida em estudo tomográfico de crânio. Obteve-se correlação de Spearman (rs) positiva significativa entre [ACTH] e P/B ao diagnóstico (rs=0,39, p=0,014) e em T360 (rs =0,35; p = 0,028). Esta correlação foi mais forte ao analisar os dados do Grupo Tri em T360 (rs=0,75, p=0,0003). No Grupo Cab + Tri em T360 não houve correlação entre ACTHe e P/B (rs=0,005, p=0,98). Foi observada correlação negativa entre ECSC e ACTH (rs=-0,39;p=0,0183) somente na análise geral dos dados. Sabe-se que em resposta ao trilostano e em pacientes hipocortisolêmicos, a [ACTH] pode aumentar. No entanto, com base nos resultados analisados,a mensuração da [ACTH] pode ter aplicação como biomarcador de aumento do tamanho do corticotropinoma, durante o monitoramento terapêutico.

Pituitary-dependent hypercortisolism (PDH) is an endocrinopathy that frequently affects dogs. It is caused by a corticotropinoma, characterized by dysregulated adrenocorticotropic hormone (ACTH)secretion. Skull imaging is indicated to characterize the tumor. However, the excessive costand inherent risks of the exam end up limiting its performance. Therefore, it is important to search for biomarkers of corticotropinoma growth that reduce the need for imaging exams. The objective of this study is to evaluate the concentration of ACTH ([ACTH]) and its relationship with pituitary size in dogs with PDH at diagnosis and after treatment with trilostane alone or associated with cabergoline, and the relationship between the Clinical Score for Cushing's Syndrome (CSCS) with these two variables. The data presented include patients treated at HCV-UFRGS, VetUnity-SP, and at the University of Bologna -Italy from a multicenter study evaluating the impact of adding cabergoline to treating HPD with trilostane. Thirty-eight newly diagnosed patients (T0) were selected and divided into the Tri (Tri Group, n = 18) and Cab+Tri (Cab + Tri Group, n = 20) groups and monitored for one year (T360). The measurement of [ACTH] followed pre-analytical care and was measured by chemiluminescence using Immulite 1000, and the cranial tomographic study defined the pituitary height/brain area ratio (P/B ratio). A significant positive Spearman correlation (rs) was obtained between [ACTH] and P/B at diagnosis (rs=0.39, p=0.014) and at T360 (rs = 0.3548; p = 0.0289). This correlation was stronger when analyzing data from the Tri Group at T360 (rs=0.75, p=0.0003). In the Cab + Tri Groupat T360, there was no correlation between ACTHe and P/B (rs=0.005, p=0.98). A negative correlation was observed between ECSC and ACTH (rs=-0.39, p=0.0183) only in the overall data analysis. It is known that in response to trilostane and hypocortisolemic patients, [ACTH] may increase. However, based on the results analyzed, the measurement of [ACTH] may have application as a biomarker of increased corticotropinoma size during therapeutic monitoring.
