[The quinolones: chemical structure, mechanism of bactericidal action and profile of clinical pharmacology]

Quinolones are an interesting antibacterial group and have multiple medical practice applications. In recent years, a series of structural analogues known as quinolones of second and third generation have been synthesized, exhibiting better bactericidal potency, a broader antibacterial spectrum and an improved pharmacokinetic profile in comparison with the first generation quinolones. In addition, as opposed to what has been observed with the first generation quinolones, bacterial resistance against these drugs occurs with a very low frequency, and when it happens it is usually of low level. The present review discusses the realtionship between chemical structure and biological activity of the different quinolones. Recent models which try to explain mechanisms that lead to the bactericidal action, as well as models related to the ways in which bacterial strains can become resistant, are also explained, Finally, some general clinical pharmacological characteristics of the new fluoroquinolones are presented
