Superficial keratectomy, limbal autotransplantation and amniotic membrane transplantation in the treatment of severe chemical burns of the eye

The aim of this study was to clinically and histopathologically evaluate a treatment of severe chemical eye injuries. Limbal autograft and amniotic membrane transplantations were combined with superficial lamellar keratectomy. Fifteen New Zealand white male rabbits were used in the experiment. General anaesthesia was induced before 2.5 N NaOH solution was applied to one eye of each animal. The surgery consisted of keratectomy of necrotic cornea and transfer of limbal graft from the contralateral eye. The eye was covered by an amniotic membrane. Infectoflam eye drops were applied 4 times daily for 4-6 weeks. Two eyes spontaneously perforated at the end of the first month. Complete re-epithelization was achieved in 68 % of the eyes. Conjunctival hyperemia and limbal ischemia were present in all eyes at 2 weeks. Other symptoms such as blepharospasm, purulent discharge and symblepharon were observed in several cases. The amniotic membrane transplantation after superficial keratectomy combined with limbal autograft transplantation after severe alkali burn significantly contributes to reparative process of the ocular surface.
