Use of active pleuroperitoneal shunt tube (Denver shunt) for idiopathic chylothrax refractory to the thoracic duct ligation with cisterna chyli ablation in a dog

A three-year-old spayed female Whippet presented with the signs of dyspnea due to severe pleural effusion. The effusion was consistent with the chlous and idiopathic chylothrax was diagnosed after ruling out the underlying diseases. The thoracic duct ligation (TDL) with cisterna chyli ablation (CCA) was performed after new methylene blue injection to the mesenteric lymph node. Because the chylous effusion persisted postoperatively, second TDL was attempted 10 days after the operation. But the other thoracic duct communication still existed after the second TDL. Therefore the active pleuroperitoneal shunt tube (Denver shunt) was also implanted after TDL. The amount of chylous effusion gradually decreased and stopped at 180 days after second surgery. No recurrence has noted for 1098 days to the present.
