The effects of Nitric Oxide Donour (L-arginine) and Nitric Oxide Synthase Inhibitors (Aminoguanidine, L-NAME) on some biochemical parameters in experimentally induced Nephrotoxicity in rabbits | Deneysel Nefrotoksisite oluşturulan tavşanlarda Nitrik Oksit Donörü (L-Arginin) ve Nitrik Oksit Sentaz İnhibitörlerinin (Aminoguanidin, L-NAME) bazı biyokimyasal parametrelere etkileri

In the present study, the effect of oral administration of nitric oxide donour (L-arginine) and selective and nonselective (AG, L-NAME) nitric oxide synthase inhibitors on some biochemical parameters were aimed to determine in gentamycine induced nephrotoxicity in rabbits. In this study 48 healthy New Zealand White Rabbits were used as material. The rabbits were divided into 8 groups [I st group (Control), II nd group (L-arginine), III rd group (Aminoguanidine (AG)), IV th group (L-NG-nitro arginine methyl ester (L-NAME)), V th group (Gentamycine (GM)), VI th group (GM+L-arginine), VII th group (GM+AG) and VIII th group (GM+L-NAME)]. To induce nephrotoxicity; gentamycine sulphate applied intramuscularly (10 mg/kg dose, three times a day) for 10 days. In addition, L-arginine (2gr/L), AG (1gr/L) and L-NAME (100 mg/L) were given orally for 10 days. Blood and urine samples were collected at 0, 1, 3, 5, 7, 10 and 15 th days of the study from all animals in all groups. Total protein, albumine, BUN, Scr and nitrite values were determined in serum samples and urine protein, glucose, GGT, creatinine, nitrite and creatinine clearance were determined in urine samples. Although biochemical parameters were not changed in groups I, II, III and IV, only serum and urine nitrite concentration increased in group II and decreased in groups III and IV. Significant increase in the BUN and serum creatinine concentrations in gentamycine applied groups V, VII and VIII, and significant decrease in the creatinine clearance values were determined. But, important changes in above parameters were not determined in group VI. Increase in the urine GGT, urine protein and urine glucose levels in all gentamycine applied (V, VI, VII and VIII) groups were determined. In addition, the most rapid changes were observed in the urine GGT concentration. Although serum and urine nitrite concentration in groups V and VI increased, it was observed to decrease in groups VII and VIII. As a result, in gentamycine induced nephrotoxicity cases, stimulation of nitric oxide release was useful, inhibition of nitric oxide was harmful. Therefore, administration of L-arginine reduce severity of nephrotoxic effects of gentamycine in the long term gentamycine required treatments.

Bu çalışma tavşanlarda deneysel gentamisin nefrotoksisitesinde nitrik oksit donörü (L-arginin) ile selektif ve nonselektif (AG, L-NAME) nitrik oksit sentaz inhibitörlerinin oral kullanımlarının bazı biyokimyasal parametrelere olan etkisinin belirlenmesi amacıyla yapıldı. Bu çalışmada 48 adet sağlıklı Yeni Zelanda beyaz tavşanı materyal olarak kullanıldı. Tavşanlar 8 gruba [I. grup (Kontrol), II. grup (L-arginin), III. grup (Aminoguanidin (AG)), IV. grup (L-NG-nitro arginin metil ester (L-NAME)), V. grup (Gentamisin (GM)), VI. grup (GM+L-arginin), VII. grup (GM+AG) ve VIII. grup (GM+L-NAME)] ayrıldı. Hayvanlara nefrotoksisite oluşturmak amacıyla 10 gün süreyle (10 mg/kg dozda günde 3 defa) kas içi gentamisin sülfat uygulandı. Ayrıca L-arginin (2gr/L), AG (1gr/L) ile L-NAME (100 mg/L) oral olarak 10 gün süreyle verildi. Bütün gruplardaki hayvanlardan çalışmanın 0., 1., 3., 5., 7., 10. ve 15. günlerinde kan ve idrar örnekleri alındı. Serum örneklerinde; Tp, albümin, BUN, Scr ve nitrit değerleri, idrar örneklerinde ise; idrar protein, glikoz, GGT, kreatinin ve nitrit ile kreatinin klirensi değerleri belirlendi. I., II., III. ve IV. gruplarda biyokimyasal parametrelerde önemli bir değişim belirlenmemesine rağmen, sadece serum ve idrar nitrit konsantrasyonları bakımından II. grupta artış, III. ve IV. grupta ise düşüşler belirlendi. Gentamisin verilen V., VII. ve VIII. gruplarda serum BUN ve Scr seviyelerinde artış, kreatinin klirensi değerlerinde ise önemli düşüşler belirlendi. Ancak VI. grupta bu parametrelerde önemli bir değişim gözlenmedi. Gentamisin uygulanan tüm gruplarda (V., VI., VII. ve VIII. grup) idrar GGT, protein ve glikoz düzeylerinde artış olduğu ancak en erken değişimin idrar GGT düzeylerinde olduğu tespit edildi. Serum ve idrar nitrit konsantrasyonlarında V. ve VI. grupta artış olmasına karşın VII. ve VIII. gruplarda ise düşüş belirlendi. Sonuç olarak; gentamisin ilişkili nefrotoksisite olgularında nitrik oksit uyarımının yararlı, nitrik oksit inhibisyonunun ise zararlı etkilerinin olduğu, bu nedenle uzun süreli gentamisin tedavisi gereken durumlarda L-arginin verilmesinin nefrotoksisitenin şiddetinin azaltılmasında yararlı olacağı kanısına varıldı.
