The occurrence of Left Displacement of Abomasum (LDA) can have major impact on the economic condition of dairy herds as regards production/reproduction, risk of culling, and all the expenses related to therapeutic investigations. The standard treatment used by the veterinary practitioners in Sardinia is open surgical correction. Cost including surgery, pre- and postoperative treatment, decreased milk production after correction, milk loss as a result of administration of antimicrobials and subsequent milk withdrawal, make more advantageous for a dairy producer the choice of selling the cow. the Authors performed the 2 step laparoscopic abomasopexy to correct LDA on 11 cows coming from a 200 cows dairy farm. Preliminary results of the present study evidence that the 2 step laparoscopic abomasopexy combines the advantages of conventional open surgery methods, such as high therapeutic safety and success rates, with financial benefits as reduction of the losses in milk production, quicker reproductive recovery and absence of the cost related to antimicrobials administration and subsequent milk withdrawal. We have also found that it is possible to save many cows which were considered poor candidates for surgery due to stress and concurrent diseases. The results of this study indicate that the 2 step laparoscopic abomasopexy may be a reasonable alternative to an open surgical approach and become a routine method to treat LDA.
