Research needs in management of obesity by severe caloric restriction

Abstract: A thorough review of existing information and clinical investigations attempted to link the sudden deaths of 17 obese people to massive, rapid weight reduction from strict adherence to a low-calorie "liquid protein" or "predigested protein" diet. The study focused on the metabolic and pathophysiological consequences (particularly cardiopathological) of total and severe caloric deprivation; the research necessary to define the adverse health effects of these regimens; the comparison of various types of low-calorie diets; the influences of age, race, sex, heredity, health status, degree of obesity, immunological competence, duration of diet, drugs, and rate of weight loss; and the identification of the factors most likely to influence the safety and efficacy of these diets. While a definite cause-effect relationship could not be established, the observations provide a basis for further investigation. The subjects died of ventricular tachydysrhythmias associated with prolonged QT intervals and low voltages. Consistent findings included a decrease in cardiac mass, attenuated fiber-size, disappearance of myofibrils and excessive lipofuchsin deposition.
