A new approach to the treatment of diabetic pregnant women; report of 479 cases seen from 1963 to 1975

The perinatal risk in pregnant diabetic women can be reduced to a level comparable to that in nondiabetic pregnant women through administration of a maximal tolerated dose (MTD) on insulin. Strict control of the maternal diabetes through the MTD (the highest quantity of insulin possible without causing hypoglycemic distrubances) reduces perinatal death rates and the incidence of serious malformations. Insulin is administered in gradually increasing dosages until the first signs of intolerance appear, then the excess is eliminated. A stable calorie content diet is established in consultation with the patient for the course of pregnancy, to assure her adherence to it. Tests were conducted from 1963 to 1975 on 479 women with either clinical or latent diabetes.
