Rickets in very low-birth-weight infants

Rickets was observed in 22 very low birthweight (VLBW) infants. Of these, 15 infants were diagnosed with active rickets and 7 with healing rickets. Treatment consisted of feeding of a soy formula supplemented with calcium, phosphorus and vitamin D, as well as additional doses of vitamin D or calcium lactate. Mean daily intakes of Ca, P, and vitamin D, and mean weight gain during hospitalization were calculated. Serum levels of Ca, P and alkaline phosphatase were measured. Prompt healing (within 6 weeks) was seen in 11 infants, and delayed healing occurred in 2 infants receiving additional vitamin D. The minimum incidence of rickets in VLBW infants may be as high as 13%. Rickets in this population may be caused by inadequate intake, absorption or metabolism of Ca, P or vitamin D. Soy formulas have been implicated in poor mineral absorption and may contribute to the development of rickets in these infants.
