Fecal steroids in diarrhea. III. Experimentally-induced travellers' diarrhea

In order to differentiate among attacks of acute diarrhea, a biological approach is being used to identify etiological agents. Fecal bile acid and neutral sterol content from six adult volunteers were studied after exposure to Escherichia coli B7a (0148:H28), a travellers' diarrhea which produces heat-labile and heat-stable enterotoxins. Consistent with results from previous studies, bile acids and cholesterol metabolites were lower during diarrhea, and excretion of acidic and neutral steroids were higher, though differences were not statistically significant. These results are in marked contrast to the gastrointestinal steroid changes noted in acute shigellosis, which is an invasive intestinal infection. The fecal steroid profile is therefore being studied further to provide a reliable means of differentiating large and small bowel diarrheas.
