Reactivity to norepinephrine and effect of sodium on blood pressure during weight loss

A 2-3 month study randomized 18 moderately obese, middle-aged men with untreated hypertension on a low energy diet into 2 groups: Group 1 maintaining its dietary Na at the pre-study level; Group 2 having dietary Na restricted. Body mass loss was equal in both groups (ea. 9 kg), but mean arterial pressure was decreased significantly only in Group 2. Heartrate also decreased significantly in both groups. Urinary norepinephrine (NE), but not plasma NE, decreased in Group 1, while the reverse was true for Group 2. Infusion of NE to Group 1 elevated the mean arterial pressure, but not in Group 2. The results indicated that, in Group 1, the lack of hypotensive response may have been due to increased vascular reactivity to NE, which may have kept the blood pressure unchanged. The blood pressure drop in Group 2 as the sympathetic nervous activity lowered was not offset by a NE reactivity increase.(wz)
