Epidemiology and ecology of gastric cancer in Costa Rica

The high incidence of gastric cancer in Costa Rica, together with suitable collections of data, affords a good opportunity for studying the origins of this disease. The incidence among males is twice as high as among females. When incidence rates are classed by either birthplace or place of residence, the highest rates are found in the central provinces. Notwithstanding a decline in mortality in recent decades, the prospects for survival of gastric cancer patients in Costa Rica have improved very little during the last 20 years; the mortality rate exceeds 60% in the first year after diagnosis. Soil characteristics subjected to stepwise multiple regression analysis revealed that the only factors found to have any significant association with differences in rates of gastric cancer were potassium content, pH, zinc content, and iron content in that order of importance. Drinking water and diet were not associated with the incidence of gastric cancer. (emc)
