A nutritional care plan was designed to assure referral of diabetic patients to clinical dietitians, to set standards for nutritional care and its documentation, and to enhance communication between inpatient and outpatient nutrition services. The plan also can be used as a teaching tool. The plan incorporates nutritional assessment, education, and follow-up, and has been in operation in a hospital since early 1982 with positive reception by clinical dietitians.
