Recent scientific findings are reviewed covering the possible association of excessive eating and drinking with the incidence of gout. Gout victims are generally large and obese. The metabolic disorder in gout is hyperuricemia, an excess of urate in blood and tissue fluids, which in some cases leads to deposition of sodium urate crystals in the joints and the subsequent inflammatory reaction of acute gouty arthritis. Purine-rich diets appear to raise serum urate levels. The energy content of the diet probably influences hyperuricemia development and alcohol intake appears to increase urate synthesis by enhancing adenine nucleotide turnover. These findings have implications for some patients in terms of correcting the causes of hyperuricemia and dietary management of gout. (wz)
