Glucose intolerance and aging - evidence for tissue insensitivity to insulin

Impaired insulin secretion and impaired tissue sensitivity to insulin may contribute to the glucose intolerance that accompanies aging. Using the hyperglycemic and euglycemic clamp techniques, the relative importance of the two aging factors was compared in 84 healthy subjects, aged 21-84. The blood glucose or plasma insulin concentration was acutely raised during the intravenous clamp infusions. Results demonstrated that the major contributor to the decreased glucose tolerance observed in older subjects was impaired tissue sensitivity to insulin. It is suggested that the site of insulin resistance is in peripheral tissues. Beta cell response to glucose did not account for the age-related decline in glucose metabolism in these subjects. The impairment in insulin action was observed in both hyperglycemic and euglycemic conditions.
