O papel de uma intervenção educacional na avaliação da exposição a agrotóxicos na área agrícola de Serrinha do Mendanha | The role of an Educational Intervention in the evaluation of exposure to pesticides in the agricultural area of the Serrinha Mendanha

A região agrícola de Serrinha do Mendanha, localizada em Campo Grande, município do Rio de Janeiro, possui características próprias em função do processo de urbanização, que podem estar relacionadas com o uso e possível intoxicação por agrotóxicos. Estudos realizados na região há mais de 10 anos demonstraram as peculiaridades de percepção de risco dos agricultores locais, bem como em relação à adoção de procedimentos corretos para a aplicação dos agrotóxicos. Esses podem ser fatores determinantes na maior vulnerabilidade dessa população ao risco de adoecimento por intoxicação. Estratégias de Intervenção Educacional tem sido apontadas como capazes de modificar padrões de Conhecimentos, Atitudes e Práticas (CAP) em relação aos agrotóxicos, o que pode efetivamente provocar a redução da exposição ocupacional e ambiental a esse grupo de agentes em comunidades agrícolas. O presente estudo teve como principal objetivo avaliar a efetividade de uma estratégia de intervenção educacional em uma população exposta aos agrotóxicos em função das modificações das pontuações CAP em três diferentes momentos: antes, logo após e 2 meses após a intervenção. Foi aplicado um questionário CAP com o objetivo de obter dados não só sócio demográficos, mas também das diferentes percepções, dos conhecimentos, atitudes e práticas dos agricultores da região em relação aos agrotóxicos. Reuniões de grupo focal foram realizadas com representantes da comunidade com intuito de construir a estratégia de intervenção baseada na participação ativa da comunidade. A análise dos dados permitiu concluir a mudança no padrão de participação da mulher nos trabalhos da lavoura e no consumo de determinados agrotóxicos. Os agricultores da região permanecem apresentando uso inadequado de EPI, fruto de uma percepção de risco própria, construída a partir de valores sociais, culturais de vivência de cada indivíduo. Apesar dos trabalhos desenvolvidos anteriormente na região, os agricultores demonstraram resistência na participação, devido ao desenvolvimento de uma descrença em atividades de pesquisa baseada no não retorno dos resultados à comunidade. As discussões do grupo focal apontaram para a importância do desenvolvimento de estratégias de intervenção educacionais constantes e com participação ativa da comunidade, tendo em vista seu potencial de transformação de conhecimentos, atitudes e práticas em populações expostas aos agrotóxicos. A Intervenção Educacional proposta foi capaz de alterar algumas pontuações CAP entre os agricultores de Serrinha do Mendanha. Foram observados aumentos significativos (p < 0,001) na pontuação CAP antes e logo após a intervenção proposta. A pontuação de conhecimentos antes da aplicação da intervenção para os homens (10,89 ± 0,94) foi significativamente maior (p < 0,05) do que para mulheres (10 ± 0,63). A análise CAP demonstrou que as atitudes nos momentos antes (5,94 ± 1,34) e logo após a intervenção (6,82 ± 1,29), entre indivíduos de menor escolaridade, apresentaram um aumento, mesmo que a níveis borderline de significância estatística (p= 0,0516; 0,05 < p < 0,1). Meeiros apresentaram pontuações CAP de atitudes maiores após a intervenção (antes - 5,75 ± 1; logo após - 6,87 ± 0,97; p < 0,05). Em relação à faixa etária, o grupo com idade inferior a 55 anos apresentou um aumento significativo da pontuação de atitude (p < 0,05) logo após a intervenção (6,77 ± 1,07) em relação ao momento anterior a intervenção (5,61 ± 0,79). Da mesma forma, os valores totais de CAP antes (25 ± 2,24) e depois (27,77 ± 2,68) apresentaram diferenças significativas (p < 0,05) nesse grupo. A construção de estratégias de Intervenção Educacional pode significar uma importante ferramenta de remodelamento de padrões CAP, o que, segundo a literatura e os achados dessa pesquisa, podem levar à reduções de exposição aos agrotóxicos em populações agrícolas.

The agricultural region of Serrinha do Mendanha, located in Campo Grande, Rio de Janeiro, has its own characteristics due to the urbanization process, which may be related to the use and possible pesticide poisoning. Some works in the area in the last 10 years have shown the peculiarities of risk perception for local farmers and the weak, when present, guidance to the correct application of pesticides. These can be determining factors in the increased vulnerability of this population to the risk of illness by intoxication. Educational Intervention strategies have been identified as able to modify patterns of Knowledge, Attitudes and Practices (KAP) in relation to pesticides, which can effectively cause the reduction of occupational and environmental exposure to this group of agents in agricultural communities. This study aimed to evaluate the effectiveness of an educational intervention strategy in a population exposed to pesticides in the light of changes in KAP scores at three different times: before, after and 2 months after the application of an Educational Intervention. A KAP questionnaire was applied in order to obtain data about not only socio-demographic, but also the different perceptions about knowledge, attitudes and practices in relation to pesticides of farmers in the region. Focus group meetings were held with community representatives with the aim of building intervention strategy based on active community participation. Data analysis concluded the change in the pattern of women's participation in the farming work and consumption of certain pesticides. Farmers in the region remain presenting inappropriate use of PPE, result of a particular risk perception, built from social, cultural experience of each individual. Despite the works previously developed in the region, farmers have shown resistance in participation at the new projects, due to a disbelief in research's activities that do not returning the results to the community. The focus group discussions pointed the importance of developing constant educational intervention strategies and active participation of the community, given its potential of transformation of knowledge, attitudes and practices in exposed populations to pesticides. Educational Intervention stra tegies have been identified as able to modify patterns of Knowledge, Attitudes and Practices (KAP) in relation to pesticides, which can effectively cause the reduction of occupational and environmental exposure to this group of agents in agricultural communities. The Educational Intervention proposal was able to change some KAP scores among farmers of the Serrinha do Mendanha. Significant increases (p < 0.001) in scoring KAP before and after the proposed intervention were observed. The score knowledge before implementing the intervention for males (10.89 ± 0.94) was significantly higher (p < 0.05) than for women (10 ± 0.63). KAP analysis showed that attitudes in the moments before (5.94 ± 1.34) and after the intervention (6.82 ± 1.29), among less educated individuals, showed an increase, even if borderline levels statistical significance (p = 0.0516; 0.05 < p <0.1). Sharecropper showed higher KAP scores attitudes after intervention (before - 1 ± 5.75; after - 6.87 ± 0.97; p < 0.05). In terms of age, the group aged less than 55 years showed a significant increase of attitude score (p < 0.05) after the intervention (6.77 ± 1.07) compared to the previous time the intervention (5.61 ± 0.79). Likewise, the total amounts of KAP earlier (25 ± 2.24) and after (27.77 ± 2.68) was significantly different in this group (p <0.05). Construction of Educational Intervention strategies could mean a major KAP standards remodeling tool, which according to the literature and the findings of this research may lead to exposure to pesticides reductions in agricultural populations.
