Mukoviszidose und Haustiere – Beurteilung des Gesundheitsrisikos durch regelmäßigen Tierkontakt für Patienten mit Mukoviszidose | Cystic Fibrosis and Pet Animals – Evaluation of the Health Risk Associated with Regular Contact with Pet Animals for Patients with Cystic Fibrosis

Die Vermeidung von Atemwegsinfektionen ist für Patienten mit Mukoviszidose (Cystischer Fibrose, CF) von essentieller Bedeutung, da jede Pneumonie die Gefahr bleibender Verschlechterung des Krankheitsstatus in sich birgt. Neben der großen Gefahr durch Infektionen gilt es grundsätzlich, den Kontakt zu allem zu meiden, was die Atemfunktion beeinträchtigt. Hier ist auch an Stäube und Allergene zu denken, wie z.B. Aspergillus spp., die eine allergische bronchopulmonale Aspergillose (ABPA) auslösen können. Da der Umgang mit Tieren generell mit einem erhöhten Vorkommen an Erregern, Stäuben und Allergenen verbunden ist, wird dieser von vielen behandelnden Ärzten für Patienten mit CF sehr kritisch gesehen. Da bisher keine Leitlinie zu dem Thema „CF und Tierkontakt“ existiert, die auf spezifischen Untersuchungen mit Patienten mit CF basiert, sollten mit der vorliegenden Studie erste Schritte unternommen werden, entsprechende Daten zu erheben. Ziel war es, das für Patienten mit CF von Haustieren ausgehende Risiko, sowohl qualitativ als auch quantitativ, zu ermitteln. Die Arbeitshypothese der Studie wurde wie folgt formuliert: Das Halten von Haustieren bzw. regelmäßiger Tierkontakt stellt für Patienten mit CF ein erhöhtes Gesundheitsrisiko dar. Zunächst sollte das Ausmaß von Tierkontakten und das damit in Zusammenhang stehende Hygieneverhalten der Patienten mit CF des Christiane Herzog-Zentrums Berlin ermittelt werden. Zu diesem Zweck wurden Fragebögen an 290 Patienten des Zentrums verteilt und die freiwillig beantworteten Fragen ausgewertet. Für weitere Analysen wurden zwei Kohorten gebildet. Zur Kohorte der Patienten mit regelmäßigem Tierkontakt (n= 75) wurden Patienten gezählt, die aktuell zehn oder mehr Stunden pro Woche Kontakt zu einem Tier haben. Patienten mit weniger oder gar keinem Umgang mit Tieren zählten zur Kohorte der Patienten ohne regelmäßigen Tierkontakt (n= 97). Die Ergebnisse der Fragebogenauswertung zeigen, dass Patienten mit CF trotz ihrer Erkrankung nicht weniger Haustiere halten als der Durchschnitt der deutschen Bevölkerung. Sie bevorzugen allerdings mehr Hunde und Katzen gegenüber sonstigen Haustieren. Das Hygieneverhalten im Umgang mit Tieren ist nicht stark ausgeprägt. Auffallend hoch ist die positive emotionale Wirkung (90%), die die tierbesitzenden Patienten ihrem Haustier zuschreiben. Im zweiten Teil der Studie wurden beide Kohorten auf gesundheitliche Unterschiede untersucht. Hierzu wurden retrospektiv klinische Daten wie die Einsekundenkapazität der Lunge (FEV1), der Body Mass Index (BMI), Exazerbations- und Hospitalisationsraten sowie mikrobiologische Daten (qualitativer und quantitativer Erregernachweis aus Sputumanalysen im Untersuchungszeitraum) miteinander verglichen. Um eine qualitativ hochwertige statistische Analyse durchzuführen, waren Datenreduktionen notwendig. Nach dem Ausschluss der Daten von insgesamt 19 Patienten (lungentransplantierte Patienten bzw. Patienten mit fehlenden Daten), kamen schließlich 56 Patienten mit regelmäßigem Tierkontakt und 58 Patienten ohne regelmäßigen Tierkontakt zur statistischen Auswertung. Die Ergebnisse der statistischen Datenauswertung geben keinen Hinweis darauf, dass Patienten mit regelmäßigem Tierkontakt mehr Infektionen haben als Patienten ohne regelmäßigen Tierkontakt. FEV1, BMI, Exazerbations- und Hospitalisationsraten unterschieden sich in beiden Patientengruppen statistisch nicht signifikant. Allerdings konnte regelmäßiger Tierkontakt als Prädiktor für eine allergische bronchopulmonale Aspergillose (ABPA) ermittelt werden. Patienten mit regelmäßigem Tierkontakt haben gegenüber Patienten ohne regelmäßigen Tierkontakt ein siebenfach erhöhtes Risiko eine ABPA zu entwickeln. Der dritte Teil der Studie diente dem Nachweis von Bakterien und Pilzen, die bei Haustieren von Patienten mit CF und in ihrem Umfeld vorkommen, der Einschätzung ihrer Pathogenität für die Patienten und der Ermittlung von Transmissionen. Dazu wurden 22 Patienten mit Haustieren zu Hause aufgesucht und 364 Proben von den verschiedenen Haustierarten und von ihrem Umfeld genommen. Die Proben wurden anschließend mykologisch und bakteriell über Kultivierung, mikroskopische Untersuchung und falls erforderlich DNA-Sequenzierung und MALDI-TOF Verfahren bis auf Speziesebene analysiert. In diesem Teil der Studie wurden mehrfach Erreger bei Patienten und ihren Haustieren nachgewiesen, bei denen aufgrund der Patienten-Haustier- Historie eine Transmission als wahrscheinlich anzusehen war. Die mykologischen Analysen zeigten, dass mit der Haltung von Tieren auf organischer Einstreu wie Heu, Stroh, Holzspäne und Rindenmulch eine große Schimmelpilzbelastung, insbesondere von Aspergillus spp., verbunden ist. Der festgestellte erhöhte Erregerdruck spiegelt sich jedoch nicht in einer Erhöhung der Infektionsraten wider, scheint also keine Auswirkungen auf die tierbesitzenden Patienten zu haben. Der Anfangsverdacht auf erhöhten Erregerdruck mit potentiell problematischen Erregern bei Reptilien, sollte in Zukunft genauer untersucht werden. Die vorliegenden Studie kann mit einer Patientenzahl von n= 4 keine aussagekräftigen Ergebnisse liefern. Für die Entwicklung einer ABPA konnten die in der vorliegenden Studie erhobenen Daten die Arbeitshypothese stützen, für alle anderen Aspekte (Infektionsrisiko, Lungenfunktion, Ernährungsstatus, Hospitalisationen und Exazerbationen) jedoch nicht.

The prevention of pulmonary infections is essential for patients with CF, as every case of pneumonia bears the risk of a consistent decrease of their respiratory and health status. Beside the risk of pathogens, every pulmonary obstruction triggered by dust or allergens must be avoided. For example, Aspergillus spp. can elicit an allergic bronchopulmonary aspergillosis in patients with CF. Since the regular contact with animals is generally associated with an increased exposure to pathogens, dust and allergens, many doctors who treat patients with CF advise against having pets. Since up to this date, there exist no guidelines concerning ”CF patients and animal contact“, which are based on specific studies with CF patients, the present study provides a first step in the acquisition of relevant data. The aim was to assess the risk associated with pet animals for CF patients both quantitatively and qualitatively. The working hypothesis guiding this study has been formulated as follows: Pet ownership and frequent contact to pets in general result in an increased health risk for patients with CF. First, the intensity and extent of pet contact and the related hygienic behavior of patients from the Christiane Herzog-Zentrum Berlin were investigated. In this part of the project, 290 Patients of the Christiane Herzog-Zentrum volunteered to fill out a questionnaire and the provided answers were evaluated. For further analyses the group of patients with CF was divided in two groups. The group with regular pet contact (n=75) consisted of patients who all had currently ten hours or more in a week contact to at least one pet. Patients with less or no regular pet contact accounted for the cohort without regular contact (n=97). The analysis of the questionnaires revealed that CF patients, despite their condition, are not less likely to own pets than the German average. CF patients do prefer dogs and cats over other pets though, more so than German pet owners without CF. The hygienic behaviour in the context of animal contact of patients with CF was not particularly pronounced. It is remarkable however, that 90% of CF patients with pets attribute a positive emotional impact to their animals. In the second part of the study the health status of both cohorts was compared. Therefore clinical data, such as the forced expiratory volume in one second (FEV1), Body Mass Index (BMI), rates in exacerbation and hospitalization, as well as microbiological data (qualitative and quantitative detection of pathogens in sputum samples taken during the study period) were retrospectively compared across cohorts. In order to perform a high-quality statistical analysis, data reductions were necessary. After the exclusion of 19 patients (lung-transplant patients, patients with missing data), 56 patients with regular animal contact and 58 patients without regular animal contact were finally accepted into the statistical analysis. Results of the statistical analysis provide no evidence that CF patients with regular animal contact suffer more infections than CF patients without regular animal contact. FEV1, BMI, rates in exacerbation and hospitalization did not significantly differ across cohorts. However, regular animal contact was a predictor for allergic bronchopulmonary aspergillosis (ABPA). CF patients with regular animal contact were 7 times more likely to develop an ABPA in comparison to CF patients without regular animal contact. The third part of the study focused on the detection of bacteria and fungi that are associated with pets and their environment, in order to look for pathogenity and possible transmissions. For that purpose, a subsection of patients with pets were visited at their homes and pets and the home environment were examined in situ. 364 samples were analysed for clinically relevant bacteria and fungi by cultivation, microscopic examination and if necessary by DNA-sequencing and the MALDI-TOF method for bacterial analysis. This part of the project revealed multiple pathogens in patients and their pets, for which, considering the patient-pet-history, a transmission has to be considered likely. The mycological analysis showed that the keeping of pets on organic litter such as hey, straw, wood shavings or bark mulch result in an increased exposure to moulds, in particular Aspergillus spp.. This documented increase in pathogen prevalence does however not coincide with an increase in infections. The initial suspicion concerning the increased prevalence of potentially problematic pathogens from reptiles should be investigated in future research. The present study with only 4 relevant cases cannot provide conclusive evidence. The results of the present study support the working hypothesis concerning the development of an ABPA, but not concerning all other aspects (risk of infection, lung function, nutritional status, hospitalizations and exacerbations).
