德语. Non-Hodgkin Lymphome (NHL) repräsentieren maligne Tumore des Immunsystems mit einer weiterhin schlechten Prognose für einen Drittel der Patienten. Die antigenpräsentierenden Humanen Leukozyten Antigene (HLA) spielen sowohl auf der Zelloberfläche, als auch als freigesetzte lösliche Moleküle eine herausragende Rolle bei der Erkennung, der Abwehr und der Flucht der Tumorzellen vor der Eliminierung durch das Immunsystem in der Tumormikroumgebung. HLA-E wird bereits als prognostischer Marker für den Krankheitsverlauf von soliden Tumoren diskutiert. Bei hämatologischen Malignitäten ist die immunbiologische Bedeutung von HLA-E zurzeit noch unzureichend untersucht. Daher war das Ziel dieser Arbeit, das Potenzial von HLA-E als prognostisches und therapeutisch relevantes Zielmolekül bei den zwei klinisch diversen NHL, der indolenten chronisch lymphatischen Leukämie (CLL) und den aggressiven B-Zell Non-Hodgkin Lymphomen (B-NHL), im Vergleich zueinander und zu gesunden Kontrollpersonen zu untersuchen. Bei den Patienten mit CLL wurden, sowohl der HLA-E*01:03 Status, als auch die damit assoziierten erhöhten sHLA-E Konzentrationen als unabhängige prädiktive Parameter für ein 5-Jahres progressionsfreies Überleben identifiziert. Funktionell hemmten sHLA-E Moleküle die NK-Zell Aktivität gegenüber Tumorzellen durch die Herunterregulation der Degranulation und IFN-γ Produktion durch NK-Zellen, und die Förderung der TGF-β Freisetzung. Bei den aggressiven B-NHL war der HLA-E*01:01 Status, der mit einer verringerten HLA-E Oberflächenexpression assoziiert ist, ein unabhängiger prädiktiver Parameter für eine komplette Remission. Insbesondere die erhöhten vesikulären sHLA-E (sHLA-E EV) Konzentrationen der Patienten zeigten einen schützenden Effekt auf das 5-Jahres Gesamtüberleben der Patienten. In dieser Arbeit wurde zum ersten Mal eine Assoziation zwischen dem HLA-E*01:01/01:03 Polymorphismusund dem sHLA-E bzw. sHLA-E EV bei B-NHL Patienten beschrieben, die unabhängige prädiktive Biomarker darstellten, und Patienten mit indolenten und aggressiven B-NHL für zusätzliche zielgerichtete immuntherapeutische Optionen identifizieren könnten.

英语. Non-Hodgkin's lymphoma (NHL) represent malignant tumors of the immune system with a remaining poor prognosis in one third of NHL patients. The antigen presenting human leukocyte antigens (HLA) play on the cell surface and released as soluble molecules a paramount role in the detection, control and escape of tumor cells against their elimination of the immune system in the tumor microenvironment. HLA-E is already discussed as a predictive marker for the course of solid tumors. In hematological malignancies the immune biological importance of HLA-E is insufficiently investigated. Therefore, the aim of this study was to evaluate the potential of HLA-E as a prognostic and therapeutic relevant target molecule in the two clinically diverse NHL, namely the indolent chronic lymphocytic leukemia (CLL) and the aggressive B cell NHL (B-NHL), in comparison of each other and to healthy individuals. In CLL, the HLA-E*01:03 status and the associated increased sHLA-E levels were identified as independent predictive parameters for 5-year progression-free survival. In view of NK cell functionality, sHLA-E molecules inhibited the NK cell acitivity against tumor cells by decreasing the degranulation activity and IFN-γ production of NK cells, and increasing the release of TGF-β. In patients with aggressive NHL, the HLA-E*01:01 status, which is associated with a decreased HLA-E expression, was an independent parameter of complete remission. In particular, the increased vesicular sHLA-E levels (sHLA-E EV) had a protective effect on 5-year overall survival of the patients. This work showed for the first time an association of the HLA-E*01:03/01:01 polymorphism and sHLA-E/sHLE-E EV levels in patients with B-NHL, which represent independent predictive biomarkers and may identify patients with indolent or aggressive B-NHL for additional targeted immune therapeutic therapy options.
