Imunoterapia oral : onde aplicar?

A dermatite atópica canina (DAc) tem sido progressivamente melhor compreendida, o que tem permitido o desenvolvimento de tratamentos mais eficazes. A imunoterapia com alergénios (ICA), reconhecida como a única forma de tratamento definitivo para a dermatite atópica, tem vindo a ser explorada através de diferentes vias de administração. Estas visam facilitar a aplicação dos alergénios, promovendo uma maior adesão por parte dos titulares, sem comprometer a eficácia terapêutica. A ICA oral surge como uma via promissora devido às caraterísticas únicas da mucosa oral; no entanto, os locais de administração podem ser distintos quanto à eficácia e segurança. Neste contexto, o presente estudo teve como objetivo identificar a região da mucosa oral mais adequada para a aplicação da ICA, considerando que áreas com maior densidade de células de Langerhans (CLs) são mais propícias ao sucesso terapêutico, enquanto que regiões com maior número de mastócitos apresentam um risco acrescido de reações adversas. Para tal, foram recrutados seis cadáveres de cães submetidos a eutanásia no Hospital Escolar Veterinário (HEV) da Faculdade de Medicina Veterinária (FMV), e realizadas biópsias de cinco regiões da mucosa oral potencialmente viáveis para a ICA - bucal, vestibular, sublingual, palatina e gengival. As amostras foram processadas no laboratório, sendo coradas com Hematoxilina-Eosina (H&E) e Azul de Toluidina a 0,5% para a identificação de mastócitos. Além disso, foi utilizada a técnica de imunohistoquímica para a identificação das CLs. Posteriormente, as lâminas foram analisadas ao microscópio, permitindo a contagem de mastócitos e CLs nas diferentes regiões. A densidade celular em cada região da mucosa oral foi calculada e estes valores foram submetidos a análise estatística. Os resultados não revelaram diferenças estatisticamente significativas na densidade de mastócitos entre as regiões (p = 0,38). No entanto, verificaram-se diferenças significativas na densidade de CLs, sendo a região bucal a que apresentou a maior densidade destas células (8,25 ± 4,06). Foram registadas diferenças significativas entre a mucosa bucal e as regiões vestibular (p = 0,043), sublingual (p = 0,039) e palatina (p < 0,001). Conclui-se, assim, que a mucosa bucal é a região mais adequada para a administração de alergénios no âmbito da ICA oral. Estes resultados abrem caminho para novas investigações, já em curso, que confirmem a eficácia clínica deste método, promovendo também uma maior adesão dos titulares ao tratamento

ABSTRACT - ORAL IMMUNOTHERAPY – WHERE TO APPLY? - Canine atopic dermatitis (DAc) has become progressively better understood, leading to the development of more effective treatments. Allergen immunotherapy (ICA), recognized as the only definitive treatment for atopic dermatitis, has been explored through different administration routes. These aim to facilitate the application of allergens, promoting greater compliance by owners, without compromising therapeutic efficacy. Oral ICA has emerged as a promising route due to the unique characteristics of the oral mucosa; however, the administration sites may differ in terms of efficacy and safety. In this context, the present study aimed to identify the most suitable region of the oral mucosa for the application of ICA, considering that areas with a higher density of Langerhans cells (CLs) are more conducive to its effectiveness, while regions with a higher number of mast cells pose an increased risk of adverse reactions. To investigate this, six dog cadavers were submitted to euthanasia at the Hospital Escolar Veterinário (HEV) of the Faculdade de Medicina Veterinária (FMV), and biopsies were taken from five potentially viable regions of the oral mucosa for ICA - buccal, vestibular, sublingual, palatal, and gingival. The samples were processed in the laboratory and stained with Hematoxylin-Eosin (H&E) and 0.5% Toluidine Blue to identify mast cells. Additionally, immunohistochemistry techniques were employed to identify CLs. The slides were subsequently analyzed under a microscope to count mast cells and CLs in the different regions. Cell density in each oral mucosa region was calculated and subjected to statistical analysis. The results did not reveal statistically significant differences in mast cell density between regions (p = 0,38). However, significant differences were observed in CL density, with the buccal region showing the highest density of these cells (8.25 ± 4.06). Were noted significant differences between the buccal mucosa and the vestibular (p = 0,043), sublingual (p = 0,039), and palatal regions (p < 0,001). In conclusion, the buccal mucosa appears to be the most suitable region for allergen administration in oral ICA. These findings pave the way for further investigations, already underway, to confirm the clinical efficacy of this method, also promoting greater adherence of the owners to the treatment
