Anatomic and Excretory Urography Investigation of the Kidneys, Ureters and Urinary Bladder in Guinea Pig

BACKGROUND: Excretory urography is a method of imaging the kidneys, ureters, and urinary bladder which uses contrast medium containing the iodine compounds.OBJECTIVES: This study aimed to evaluate the anatomical structures of urinary tract in the nephrogram, pilogram, and cystogram phases, and determine the exact standard for the size of kidneys, ureters and urinary bladder in guinea pigs to be used to interpret the results, and clinical decisions.METHODS: This study was carried out on 10 guinea pigs with a mean age of 12±1.33 months and average weight of 1.12±0.18 kg. Before to the administration of contrast medium, each guinea pig was fast and Dimethicone 20 mg/kg was given orally. At the time of administration of contrast agent, each animal was sedated by using Ketamine 30 mg/kg and diazepam 5 mg/kg cocktail, and then 1500 mgI/kg of meglumine compound 60 % was injected subcutaneously over the shoulder area. Ventrodorsales and lateral abdominal X-rays were taken, thereafter every 5 minutes up to 60 minutes to complete the pylogram phase. In lateral radiographs of each guinea pig, the length of the body of the second lumbar vertebra was measured to be used as an indicator in determining the standard size of the kidneys.RESULTS: Based on the results of this study, the average length, width, and thickness of the right kidney compared to the length of the second lumbar vertebra were 2.19, 1.64, and 1.33 cm, and in the left kidney of 2.09, 1.53, and 1.41 cm and this average in right and left ureter was 6.41 and 6.22 cm, respectively.CONCLUSIONS: The exact standards can be used in the interpretation of results, and clinical decisions to determine the normal and abnormal size of kidneys, ureters and bladder in the guinea pigs.

Ureterocolonic anastomosis was evaluated in 13 clinically normal dogs. Urinary continence was maintained after surgery, and the procedure was completed without technique errors in all but 2 dogs. Three dogs died within 5 weeks (2 of undetermined causes and 1 of aspiration pneumonia and neurologic disease), and 1 dog was euthanatized 4 months after surgery because of neurologic signs. Two healthy dogs were euthanatized 3 months after surgery for light microscopic evaluation of their kidneys. Five dogs were euthanatized 6 months after surgery for light microscopic evaluation of their kidneys. Gastrointestinal and neurologic disturbances developed in 4 dogs at various postoperative intervals. Plasma ammonia concentration measured in 2 dogs with neurologic signs was increased. Plasma ammonia concentration measured in 5 dogs without neurologic signs was within normal limits. All 5 dogs, in which metabolic acidosis was diagnosed, had high normal or above normal serum chloride concentration. Serum urea nitrogen values were increased after surgery because of colonic absorption of urea. Serum creatinine concentration was increased in 1 dog 6 months after surgery. Individual kidney glomerular filtration rate was reduced in 38% (3/8) of the kidneys from 4 other dogs at 6 months after surgery. Of 5 dogs euthanatized at 3 to 4 months after surgery, 4 had bilateral pyelitis, and 1 had unilateral pyelonephritis. Six months after surgery, pyelonephritis was diagnosed in 40% (4/10) of the kidneys from 5 dogs. The ureterocolonic anastomosis procedure is a salvage procedure that should allow complete cystectomy. However, variable degress of metabolic acidosis, hyperammonemia, and neurologic disease may result.

OBJECTIVE To determine whether passive ureteral dilation (PUD) would occur after an indwelling ureteral stent was left in place in healthy dogs for 2 or 6 weeks, ureteroscopy would be possible at the time of stent removal, and PUD would be reversible after stent removal. ANIMALS 5 healthy adult female Beagles. PROCEDURES A ureteral stent was cystoscopically placed in each ureter of each dog with fluoroscopic guidance (week 0). One stent was removed from 1 ureter in each dog after 2 weeks (ureter group 1), and the other was removed after 6 weeks (ureter group 2); removal timing was randomized. Computed tomographic excretory urography was performed every 2 weeks from weeks 0 through 10 to measure ureteral diameters. Ureteroscopy was attempted at the time of ureteral stent removal in each group. Ureteral diameters were compared among measurement points. RESULTS The degree of PUD was significant after 2 and 6 weeks of stent placement in both ureter groups. Mean diameter of the midportion of the ureter in both groups prior to stent placement was 1.70 mm (range, 1.3 to 2.7 mm). At stent removal, mean diameter of the midportion of the ureter was 2.86 mm (range, 2.4 to 3.1 mm) in group 1 and 2.80 mm (range, 2.1 to 3.4 mm) in group 2. Ureteroscopy was successfully performed in all dogs up to the renal pelvis. Compared with week 0 values for diameter of the midportion of the ureter, the degree of PUD induced by stent placement had reversed by week 8 in group 1 (mean diameter, 2.00 mm [range, 1.5 to 2.3 mm]). CONCLUSIONS AND CLINICAL RELEVANCE Results indicated that ureteral stent placement for 2 weeks would result in sufficient PUD in healthy dogs to allow ureteroscopy at the time of stent removal and that the original ureteral diameter would eventually be restored. Additional research is needed to determine whether findings would be similar for dogs with urinary tract disease.

Objective-To determine the prevalence of circumcaval ureters and other caudal vena cava variations in cats and determine whether circumcaval ureters were associated with macroscopic evidence of ureteral obstruction. Sample-301 domestic cat cadavers obtained from an animal shelter. Procedures-All cat cadavers were examined, and anatomic variations of the ureters and caudal vena cava were recorded. In cadavers with a circumcaval ureter, kidney length, width, and height were measured, and the ureters were examined macroscopically to determine whether there was gross evidence of ureteral obstruction in cats with circumcaval ureters. Results-At least 1 circumcaval ureter was present in 106 of the 301 (35.2%) cats, with a right circumcaval ureter identified in 92 (30.6%) cats, a left circumcaval ureter identified in 4 (1.3%), and bilateral circumcaval ureters identified in 10 (3.3%). Twenty-one (7.0%) cats had a double caudal vena cava, including 2 cats in which the double caudal vena cava was the only anatomic abnormality identified. No sex predilection for anatomic abnormalities was found. Mean right kidney length was significantly greater than mean left kidney length in cats with a right circumcaval ureter. Conclusions and Clinical Relevance-Circumcaval ureter was present in approximately a third of cats in this study. Variation in the development of the caudal vena cava is the proposed cause. The clinical relevance of this variation is unknown.

We determined the position, dimensions, and structure of the kidneys, ureters, bladder, and urethra of 62 female sheep by use of ultrasonography. A 5.0-MHz convex transducer was placed over the right flank to examine the kidneys, and a 5.0 MHz-linear transducer was used to examine the bladder and urethra transrectally. All examinations were performed on sheep in standing position. The left kidney was 7.1 to 8.9 cm long, 3.4 to 5.5 cm wide, and 3.3 to 4.7 cm deep. Diameter of the parenchyma and renal sinus of the left kidney ranged between 1.1 and 1.9 cm and 1.1 and 2.0 cm, respectively. Circumference of the medullary pyramids varied between 2.1 and 3.3 cm. Similar ultrasonographic measurements were obtained for the right kidney. The diameter of the bladder varied between 0.3 and 6.9 cm in 96.8% of the sheep. The diameter of the bladder could not be determined in 32% of the sheep because it was > 10 cm, and, therefore, was beyond the penetration depth of the scanner. The only part of the urethra that could be ultrasonographically visualized was the internal urethral orifice. It had diameter between 0.1 and 0.2 cm. The ureters could not be ultrasonographically visualized in any of the sheep examined. The urinary tract of 8 sheep was examined 10 times within 2 weeks to examine whether measurements were reproducible. The interassay variation coefficient determined ranged from 3.1 to 31.8%, although for most variables, it ranged between 5 and 11%. Measurements for the length and width of the kidneys had the smallest interassay variation coefficient, whereas values obtained for diameter of the bladder and urethra, as well as thickness of the bladder, had the largest. It was concluded that the ultrasonographic findings described in this study can be used as references for diagnosis of morphologic changes in the kidneys, bladder, and urethra of sheep.

Realizou-se, em sete cães adultos, o autotransplante renal esquerdo associado à nefrectomia contralateral para avaliação da técnica de ureteroneocistostomia extravesical modificada pela sondagem ureterovesical peroperatória. Durante a sutura do ureter na bexiga, foi mantida uma sonda uretral na região da anastomose ureterovesical, o que facilitou a realização da técnica cirúrgica e permitiu a confecção de anastomose de diâmetro adequado. A avaliação do rim transplantado e do ureter correspondente foi feita mediante ultra-sonografia a cada sete dias durante as seis primeiras semanas de pós-operatório, período em que também se fez o exame clínico diário de todos os animais. No último dia (42º) da avaliação a curto prazo, realizou-se a urografia excretora. Em seguida, manteve-se acompanhamento clínico periódico dos sete cães por um período mínimo de quatro meses, sem que fosse detectada qualquer alteração digna de nota. A técnica de reconstrução do trato urinário utilizada mostrou-se eficiente, não se observando nenhuma complicação urológica no pós-operatório de todos os animais.

Ureteral ectopy is a rare disorder in the small animals’ clinic. It is characterized as a congenital anomaly, resulting from the ducts differentiation failure during embryogenesis. In this scenario, the ureters present themselves outside the anatomical site, being inserted into the uterus, urethra, urinary vesicle neck, or vagina. The clinical signs are urinary incontinence and perivulvar dermatitis. Surgery is the accepted treatment to correct the anomaly. The surgical procedure is based on relocating the ectopic ureter and treating associated modifications. This report describes a case of intramural bilateral ureteral ectopy, corrected surgically through the neoureterocystostomy technique, making it possible to control the animal’s urinary incontinence.

To test a modified extravesical ureteroneocystostomy using a urethral probe like a stent just on the peroperative time, seven adult mongrel dogs underwent bilateral nephrectomy followed by unilateral autotransplantation. The other kidney was discarded. The evaluation of the animals was made by ultrasonography and clinical exams for the first six weeks after the surgery. The excretory urography was made on the end of this period. Then, the clinical exams were done by at least four months in each dog, without any complication.
