Cardiopulmonary effects of positive end-expiratory pressure in anesthetized, mechanically ventilated ponies

To investigate the cardiopulmonary effects of positive end-expiratory pressure (PEEP), values of 10, 20, and 30 cm of H2O, were applied to anesthetized, dorsally recumbent, ventilated ponies. After IV induction of general anesthesia, PEEP was superimposed on controlled ventilation with 100% oxygen, and changes in gas exchange and cardiac function were measured. Increasing values of PEEP in these ponies caused a linear increase in the mean (+/- SEM) functional residual capacity, from a control value (zero end-expiratory pressure) of 1.7 +/- 0.24 L to 2.2 +/- 0.31, 2.9 +/- 0.32 and 3.4 +/- 0.3 L at PEEP of 10, 20, and 30 cm of H2O, respectively (P < 0.05). Paralleling these changes, intrapulmonary shunt fraction decreased significantly (P < 0.05) from a control value of 12.9 +/- 0.5%, to 7.5 +/- 1.1 and 2.1 +/- 0.6%, at PEEP of 20 and 30 cm of H2O, respectively. Cardiac output was decreased by increasing values of PEEP, from control value of 11.7 +/- 1.56 L/min to 9.9 +/- 1.51, 8.8 +/- 1.33 and 5.62 +/- 0.56 L/min at PEEP of 10, 20, and 30 cm of H2O, respectively. Related to decreasing cardiac output, tissue oxygen delivery also decreased as PEEP was increased, from control value of 2.0 +/- 0.09 L/min to 1.8 +/- 0.07, 1.6 +/- 0.06, and 1.03 +/- 0.04 L/min at PEEP of 10, 20, and 30 cm of H2O, respectively. Thus, the effects of increasing values of PEEP in these ponies included increased functional residual capacity and arterial oxygenation, but marked reduction in cardiac output, resulting in no improvement or decrease in total oxygen delivery. Although PEEP is useful for improving arterial oxygenation, the deleterious cardiovascular effects should be anticipated or ameliorated by use of volume loading and/or inotrope administration.

A study was designed to determine whether phenytoin (PHE) prevents the myocardial necrosis and subsequent fibrosis produced by isoproterenol (ISO). Seven groups of female rats of the Wistar strain were used. Rats in groups 1 and 5 served as controls. Rats in group 3 were injected SC with 85 mg of ISO/kg of body weight for 2 consecutive days. Rats in groups 2 and 6 received 100 mg of PHE/kg orally. Rats in groups 4 and 7 received both PHE and ISO. There were 6 to 9 rats/group. Effects of ISO and PHE were evaluated gravimetrically, histologically, and electrocardiographically. Heart weight/body weight ratios for each group receiving ISO, with or without PHE, were greater than for groups not receiving ISO (P < 0.05). Light microscopic examination of heart sections of rats given ISO alone or ISO + PHE revealed multiple and diffuse areas of fibrosis. Fibrosis in hearts from rats receiving PHE + ISO was less severe than that in hearts from rats receiving ISO alone, but the difference was not statistically significant. Electrocardiographic changes of statistical significance were not observed in rats receiving any compound (alone or in combination), when compared with the control groups of equal age.

Cardiopulmonary effects of IV administered butorphanol tartrate (BUT) were assessed in 7 yearling steers medicated with atropine and anesthetized with guaifenesin, thiamylal sodium, and isoflurane in O2 for surgical placement of duodenal cannulae. Heart rate, respiratory rate, arterial blood pressures, pHa, PaCO2, PaO2, arterial [HCO3-], esophageal temperature, and end-tidal isoflurane concentrations were measured before and after IV administration of BUT (10 mg). Mean respiratory rate increased significantly (P < 0.05) only at 45 and 60 minutes after BUT administration. Mean respiratory rate was 26 +/- 6.3 breaths/min before BUT administration and 46 +/- 12.1 breaths/min 60 minutes after BUT administration. Arterial blood pressures were increased significantly (P < 0.05) at all times, except 5 minutes after BUT administration. The mean value for mean arterial pressure was 76 +/- 9.6 mm of Hg before BUT injection and 117 +/- 12.6 mm of Hg 60 minutes after BUT injection. Mean values for pHa and arterial [HCO3-] were significantly (P < 0.05) higher at 60 minutes after BUT administration (baseline, pH = 7.25 +/- 0.04 and [HCO3-] = 29.9 +/- 3.5 mEq/L; 60 minutes after BUT, pH = 7.28 +/- 0.03 and [HCO3-] = 33.0 +/- 1.8 mEq/L). Although some statistically significant changes were recorded, IV administration of BUT to these steers did not have a marked effect on the cardiopulmonary variables measured.

The hepatobiliary dynamics of a 99mTc-labeled derivative of iminodiacetate were investigated in 29 healthy dogs. A 2-compartment model proved to be adequate to describe the hepatic time-activity curve. Model-derived variables for the hepatic accumulation and the biliary excretion and transport were used as a reference for evaluation of a number of commonly used measurements directly derived from hepatic and bilary time-activity curves (graphic variables). The difference between t50(ex) and t95(ex), representing the moments when 50 and 95%, respectively, of the maximal count rate during the hepatic excretory phase were measured, proved to be an adequate graphic variable to quantitate biliary excretion. The use of other graphic variables to quantitate hepatobiliary functions seemed unjustified.

Postnatal ossification of the atlas and axis was studied in Miniature Schnauzers by examining alizarin-stained bone clearings, cleaned dry bones, radiographs, histologic sections, and arterially perfused bone clearings. Sixty-two pups (1 day to 16 weeks old) and 4 adults were examined. In 1-day-old pups, the atlas consisted of 3 separate ossification centers: a left and right neural arch center and midventrally, the intercentrum 1, which formed the body of the atlas. The axis contained 4 separate ossification centers: a left and right neural arch center; centrum 2 in the main part of the axis body; and centrum 1, which formed the caudal part of the dens and the cranial part of the axis body. By 6 weeks of age, the epiphysis on the caudal end of the axis body had begun to ossify. At this time, the intercentrum 2, which developed as a cuboidal ossification center intercalated between centrum 1 and centrum 2 in the middle of the cranial half of the axis body, also began to ossify. The centrum of the proatlas, which formed the apex of the dens, was first seen ossified in a 9-week-old pup. These 10 ossification centers were seen as constant and separate elements. In all dogs, the dens developed from 2 separate ossification centers: the centrum of the proatlas formed the cranial one-quarter, and centrum 1 formed the caudal three-quarters. Dens dysplasia is unlikely to be a result of failure of development of one of the ossification centers for the dens. Seemingly, vascular-related ischemia may lead to postnatal resorption of at least the middle part of the dens and result in dens dysplasia, with subsequent atlantoaxial subluxation. This condition in dogs may be a suitable animal model for the study of the pathogenesis of odontoideum and related conditions in man.

Results of a prospective serologic and virologic study of ruminant livestock in Central America and the Caribbean islands revealed bluetongue virus (BTV) to be enzootic in the 9 countries participating in the study. Bluetongue virus serotypes 1, 3, 6, and 12 were isolated from sentinel animals. To the authors' knowledge, these are the first isolations of BTV from the region studied and the first isolations of these serotypes in the Western Hemisphere. Clinical disease attributable to BTV infection was not observed in sentinel animals. The incidence pattern, with respect to age and geographic location, was determined. The need to evaluate the epizootiologic features of arthropod-borne viruses (arboviruses) on a regional ecologic basis is stressed.

Fourteen ponies and 3 horses were inoculated with Ehrlichia risticii 2 to 20 months after a similar initial inoculation. Although all 17 had clinical signs of equine ehrlichial colitis after the first inoculation, 16 of 17 remained clinically normal following the second inoculation. The remaining pony had a transient fever and developed signs of depression. Before the initial inoculation, none of the animals had a detectable antibody titer to E risticii. All animals developed titers after the initial infection; however, a significant change of titer did not develop after reinoculation in most animals.

The efficacy of an ophthalmic ointment containing benzathine cloxacillin for treatment of infectious bovine keratoconjunctivitis was determined in 2 experiments. In the first experiment, Holstein calves (n = 6/group) were inoculated with Moraxella bovis and treated on postinoculation days 3 and 6 with either topically applied benzathine cloxacillin (250 mg/eye) or long-acting oxytetracycline formulation (20 mg/kg of body weight, IM). A third group of inoculated calves remained untreated as controls. For the second experiment, 4 groups of calves (n = 6/group) were inoculated and treated on postinoculation days 3 and 6 with 50, 125, 250, or 375 mg of benzathine cloxacillin; a fifth untreated group served as controls. Ocular specimens were obtained for microbiologic culture, and eyes were observed and assigned a clinical score daily. Eyes were photographed on alternate days. Ulcer surface area was measured, using a planimeter. In experiment 1, the week-2 ulcer surface area measurements for both groups of treated calves were smaller than those for controls. There was a greater frequency of M bovis isolation from the ocular secretions of controls than from those of benzathine cloxacillin-treated calves during postinoculation weeks 2 and 3. The number of M bovis isolations from the benzathine cloxacillin- and oxytetracycline-treated calves was not significantly different at any sample collection interval. On week 3, the scores of the benzathine cloxacillin-treated calves were smaller than those of controls. In experiment 2, calves of the 250- and 375-mg group had smaller scores than did controls. During postinoculation weeks 1 through 3, M bovis was isolated less frequently from the ocular secretions of calves of the 250- and 375-mg groups than from those of the control calves.

An ion chromatographic method was used to simultaneously determine nitrate and nitrite ions in biological samples. Ultrafiltration was used to produce a protein-free filtrate. Chloride interferences were eliminated by precipitation as the silver salt. Detection limits and average recoveries were 0.5 mg/L and 102% for nitrate and 0.2 mg/L and 78% for nitrite, respectively. Nitrate concentration was 2.1 +/- 1.8 mg/L and 4.9 +/- 0.8 mg/L in serum and ocular fluid of healthy cattle, respectively; nitrite was not detected. A severe case of nitrate poisoning in cattle was described and used to study the concentrations of nitrate and nitrite in samples obtained under natural conditions. Nitrate concentration of acutely poisoned cattle was 35% lower in ocular fluid at 158.1 +/- 51.4 mg/L, than in serum at 256.3 +/- 113.4 mg/L. Nitrite was not detected, because of the long processing time (> 3 hours) required for samples obtained in the field. A gradual decrease in ocular fluid nitrate of 29.4% at 24 hours, 25.9% at 36 hours, 51.6% at 48 hours, and 73.2% at 60 hours was observed; however, concentrations remained diagnostically significant (73.2 mg/L) 60 hours after death. Twenty-four hours after poisoning, the serum nitrate concentration of severely ill (52.7 +/- 51.9 mg/L) and moderately affected (12.4 +/- 5.7 mg/L) cattle that survived was indicative of the severity of clinical signs previously observed. Nitrate in serum and ocular fluid was stable in samples stored for 24 hours at 23 C, 1 week at 4 C, and 1 month at -20 C.

Thyroxine (T4), 3,5,3'-triiodothyronine (T3), and cortisol frequently are quantified in canine serum or plasma samples to aid in the diagnosis of hypothyroidism, hypoadrenocorticism, and hyperadrenocorticism. Many laboratories have established reliable references values for concentrations of these hormones in blood of clinically normal animals. However, nonpathologic factors that affect thyroidal and adrenocortical secretion may lead to misinterpretation of test results when values for individual animals are compared with reference values. The objective of the study reported here was to identify effects of age, sex, and body size (ie, breed) on serum concentrations of T3, T4, and cortisol in dogs. Blood samples were collected from 1,074 healthy dogs, and serum concentrations of the iodothyronines and cortisol were evaluated for effects of breed/size, sex, and age. Mean (+/- SEM) serum concentration of T4 was greater in small (2.45 +/- 0.06 microgram/dl)- than in medium (1.94 +/- 0.04 microgram/dl)- or large (2.03 +/- 0.03 microgram/dl)-breed dogs, the same in females (2.11 +/- 0.04 microgram/dl) and males (2.08 +/- 0.04 microgram/dl), and greater in nursing pups (3.04 +/- 0.05 microgram/dl) than in weanling pups (1.94 +/- 0.05 microgram/dl), rapidly growing dogs (1.95 +/- 0.04 microgram/dl), and young adult (1.90 +/- 0.06 microgram/dl), middle-aged adult (1.72 +/- 0.05 microgram/dl), or old adult (1.50 +/- 0.05 microgram/dl) dogs. Dogs > 6 years old had lower mean serum T4 concentration than did dogs of all other ages, except middle-aged adults. Mean serum T3 concentration in medium-sized dogs (1.00 +/- 0.01 ng/ml) was greater than that in small (0.90 +/- 0.01 ng/ml)- and large (0.88 +/- 0.01 ng/ml)-breed dogs. Serum T3 concentration was lowest in nursing (0.85 +/- 0.01 ng/ml) and weanling (0.77 +/- 0.02 ng/ml) pups, increased in rapidly growing dogs (0.99 +/- 0.01 ng/ml) and young adult dogs (1.10 +/- 0.04 ng/ml), and decreased slightly in middle-aged (0.98 +/- 0.02 ng/ml) and old (1.01 +/- 0.03 ng/ml) adult dogs. Serum T3 concentration was unaffected by sex. Mean serum cortisol concentration was greater in small (1.06 +/- 0.07 microgram/dl)- than in large (0.79 +/- 0.03 microgram/dl)-breed dogs. Serum from nursing pups 0.57 +/- 0.04 microgram/dl) contained less cortisol than did serum from older dogs (mean values greater than or equal to 0.92 microgram/dl). Serum cortisol concentration was not different between males and females. These effects of breed/size and age on serum T3, T4, and cortisol concentrations should be considered when evaluating thyroid and adrenocortical functions in dogs.
