Characterization of ambient carbon monoxide and PM 2.5 effects on fetus development, liver enzymes and TSH in Isfahan City, central Iran

Ambient carbon monoxide (CO) and particulate matters (PMs) are two important air pollutants in urban areas with known impacts on fetuses. Hence, this study measured some biochemistry factors of 200 neonates with birth dates from January 19 to October 12, 2020, including the birth weight and height and the serum levels of ALT, AST, ALP, GGT, and TSH. The Support Vector Machine-fitted land-use regression approach was used to predict the spatio-temporal variability of intra-urban PM 2.5 and CO concentrations by month during the pregnancy period of the cases employing 5 variables of Digital Elevation Model (DEM), slope, and distance from Compressed Natural Gas (CNG) stations, Bus Rapid Transit (BRT) stations, and mines and industries. Spearman correlation analysis (p < 0.05) was performed between the neonate indices and mean monthly PM 2.5 and CO concentrations at the exact residential address of maternal cases and their nearby areas in 250, 500, 1000, 1500, and 2000 m-radius buffer rings. All modeling efforts succeeded in predicting CO and PM 2.5 levels with acceptable adjusted r² values. Northern Isfahan had relatively higher CO and PM 2.5 concentrations due to its adjacency to low-vegetated open lands and its high traffic load as compared to southern areas. The correlation results between the neonate biochemistry indices and mean PM 2.5 and CO concentrations were mostly positive in most buffer rings, especially in the >500 m-radius buffer rings for PM 2.5 and in the 2000 m-radius rings for CO. Although the correlation results of PM 2.5 followed a detectable trend in the buffer rings, the associations between CO and the neonate biochemistry indices differed significantly between the buffer rings. Results showed that increasing mean monthly concentration of CO and PM 2.5 may stimulate further production of liver enzymes while decreasing the birth weight and height.

Isoflavones (ISOs) are naturally occurring endocrine-disrupting compounds. Few human studies have evaluated the effects of ISO exposure on neonatal anthropometry. This study aimed to examine the associations of maternal soy product consumption and urinary ISO concentrations, including genistein, daidzein, glycitein, and equol, with neonatal anthropometry, based on a Chinese cohort study. In Shanghai-Minhang Birth Cohort Study, pregnant women at 12–16 weeks of gestation were recruited, and they completed a structured questionnaire to assess soy product consumption during pregnancy. They also provided a single spot urine sample for the ISO assay. Neonatal anthropometric indices (birth weight; arm, waist, and head circumference; and triceps, back, and abdominal skinfold thickness) were measured at birth. Multivariable linear regression analysis was performed among the 1188 mother-infant pairs to examine the associations between maternal soy product consumption and neonatal anthropometry. The same statistical model was applied to examine the associations between maternal ISO exposure and neonatal anthropometry among 480 mother-infant pairs. Neonate girls born to mothers who “sometimes” and “frequent” consumed soy products had 169.1 g (95% confidence interval [CI], −68.9–407.1) and 256.5 g (95% CI, 17.1–495.8) higher birth weight, respectively, than those born to mothers who “never” consumed soy products during pregnancy. We observed consistent associations between higher maternal urine ISO concentrations and increased anthropometric indices (birth weight, arm and waist circumference, and triceps and abdominal skinfold thickness) in neonate girls, while no association was observed among boys. The findings suggested that maternal dietary ISO intake during pregnancy is associated with fetal development in a sex-specific pattern. In addition, follow-up studies are required to evaluate whether the observed changes in anthropometric indices at birth are associated with health conditions later in life.

The Household Air Pollution Intervention Network trial is a multi-country study on the effects of a liquefied petroleum gas (LPG) stove and fuel distribution intervention on women's and children's health. There is limited data on exposure reductions achieved by switching from solid to clean cooking fuels in rural settings across multiple countries. As formative research in 2017, we recruited pregnant women and characterized the impact of the intervention on personal exposures and kitchen levels of fine particulate matter (PM₂.₅) in Guatemala, India, and Rwanda. Forty pregnant women were enrolled in each site. We measured cooking area concentrations of and personal exposures to PM₂.₅ for 24 or 48 h using gravimetric-based PM₂.₅ samplers at baseline and two follow-ups over two months after delivery of an LPG cookstove and free fuel supply. Mixed models were used to estimate PM₂.₅ reductions. Median kitchen PM₂.₅ concentrations were 296 μg/m³ at baseline (interquartile range, IQR: 158–507), 24 μg/m³ at first follow-up (IQR: 18–37), and 23 μg/m³ at second follow-up (IQR: 14–37). Median personal exposures to PM₂.₅ were 134 μg/m³ at baseline (IQR: 71–224), 35 μg/m³ at first follow-up (IQR: 23–51), and 32 μg/m³ at second follow-up (IQR: 23–47). Overall, the LPG intervention was associated with a 92% (95% confidence interval (CI): 90–94%) reduction in kitchen PM₂.₅ concentrations and a 74% (95% CI: 70–79%) reduction in personal PM₂.₅ exposures. Results were similar for each site. The intervention was associated with substantial reductions in kitchen and personal PM₂.₅ overall and in all sites. Results suggest LPG interventions in these rural settings may lower exposures to the WHO annual interim target-1 of 35 μg/m³. The range of exposure contrasts falls on steep sections of estimated exposure-response curves for birthweight, blood pressure, and acute lower respiratory infections, implying potentially important health benefits when transitioning from solid fuels to LPG.

The presence of polycyclic aromatic hydrocarbons (PAHs) in the fetal environment is a high-priority concern due to the fetus being more sensitive than adults to these ubiquitous xenobiotics. The aim of the present study was to compare the maternal and fetal serum levels of ΣPAHs and their effects on fetal growth in an industrial and an urban area in Southwest Iran. The industrial area was the petrochemical and gas area (PGA) of the Central District of Asaluyeh County and the urban area (UA) was the Central District of Bushehr County, Ninety-nine maternal serum (MS) and 99 cord serum (CS) samples from the PGA and 100 MS and 100 CS samples from the UA were collected during May 2018 to February 2019. The mean concentrations of ΣPAHs were significantly (p < 0.05) higher in the PGA than the UA in both MS (157.71 vs. 93.56 μg/L) and CS (155.28 vs. 93.19 μg/L) samples. Naphthalene (NAP) was the predominant PAH detected in all the studied samples. Significant negative associations were found between birth weight and anthracene (ANT) level in MS (β = −22.917, p = 0.032; weight decrement = 22.917 g for a 1 μg/L increase in ANT); head circumference and chrysene (CHR) level in MS (β = −0.206, p = 0.023; head circumference decrement = 0.206 cm for a 1 μg/L increase in CHR); and birth height and NAP level in CS (β = −0.20, p = 0.005; height decrement = 0.20 cm for a 1 μg/L increase in NAP). Maternal diet had a significant effect on the serum levels of PAHs. The results of this study showed that transmission of PAHs from mother to fetus through the cord blood is an important issue and mothers who live in industrial areas and consume PAH-containing foodstuffs, and their fetuses, are more at risk than those living in a non-industrial urban area.

Bisphenol A (BPA) is one of the organic compounds that might interfere with estrogenic receptors, which would make difficulties in pregnancy hormones and fetal growth. Human chorionic gonadotropin (β-hCG) is one of the important pregnancy hormones that might be affected by environmental pollutants. The aim of this study is to investigate the probable impacts of maternal exposure to BPA on anthropometric measures of newborns. This cross-sectional study was conducted in 2019–2020 in Isfahan, Iran. During the first trimester of pregnancy, we measured the urinary BPA concentration and serum β-hCG level of 120 pregnant women, who were randomly selected from participants of a birth cohort. BPA concentration was measured using gas chromatography–mass spectrometry (GC-MS). Serum blood sample was derived and used for β-hCG analysis. Anthropometric measurement of neonates was conducted at the time of birth. BPA and β-hCG level were grouped by quartiles, and their associations with birth weight, height, and head circumference were tested using multiple linear regression model. The adjustment was done for urine creatinine, gender, and gestational age, as well as maternal age, body mass index, and education level. Data of 119 pairs of mothers and infants were available for the present study. The mean (SD) age of mothers was 29.19 (5.75) years; 56.3% of newborns were boys. Geometric mean of urinary BPA and β-hCG concentrations were 0.36 ng/g crea. (creatinine) and 17736 mIU/ml, respectively. Across the BPA tertiles, the differences in mean values were not significant for none of the anthropometric measurements and gestational age (GA). Furthermore, no significant association existed between unadjusted and adjusted tertiles of BPA and β-hCG with abovementioned birth outcomes. It seems that the non-significant association found in this study is because of low levels of urinary BPA levels than in other studies; the adverse effects on infants might be related to high concentration of BPA passed from placenta. Future longitudinal studies with large sample size are necessary to document the adverse health effects of maternal exposure to endocrine disruptor chemicals including BPA.

Parabens are a group of antimicrobial preservatives applied in an extensive range of products and are suspected to impair fetal growth because of their disrupting effect on the endocrine system. We aimed to examine maternal urinary paraben concentrations and their neonates’ outcome indexes. Methylparaben (MP), ethylparaben (EP), propylparaben (PP), and butylparaben (BP) concentrations were measured in 105 maternal urine samples collected before delivery. Length, weight, and head circumference at birth were extracted from the mothers’ delivery files. A multivariable linear regression analysis was performed to evaluate the association between paraben levels and neonatal anthropometric indices. The median levels of urinary parabens, especially BP, were higher than those in other countries. Prenatal urinary concentration of MP and BP showed a significantly positive association with birth weight in all neonates (β = 0.79, 95% CI: 0.16, 1.41, and β = 8.56, 95% CI: 3.95, 13.17, respectively), while these chemicals showed a significant negative association with head circumference (β = − 0.002, 95% CI: − 0.004, − 0.000, and β = − 0.016, 95% CI: − 0.030, − 0.002, respectively). A significant positive association between MP and birth length was also found (β = 0.004, 95% CI: 0.00, 0.00) in all the neonates. In sex-stratified adjusted models, MP and BP were found to be associated, respectively, with higher birth length and weight in male neonates (β = 0.008, 95% CI: − 0.001, 0.017, and β = 7.948, 95% CI: 1.045, 14.851). In girls, maternal urinary MP, PP, and BP were associated with increased birth weight (β = 0.831, 95% CI: 0.043, 1.620; β = 4.178, 95% CI: 0.877, 7.480; and β = 10.821, 95% CI: 3.545, 18.097, respectively), and MP and BP were associated with reduced head circumference at birth (β = − 0.003, 95% CI: − 0.005, − 0.001, and β = − 0.035, 95% CI: − 0.055, − 0.016). These results revealed potential impacts between neonatal growth and maternal exposure to parabens. However, these findings should be interpreted while considering the limitations of the present study.

Among residents living close to agricultural fields who are potentially exposed to pesticides, pregnant women and their fetuses are of particular concern for their vulnerability to environmental chemicals. In this collaborative multicenter study, we covered a wide distribution of participants in the most important fruit production zone of north Patagonia (Argentina) to investigate whether maternal residential proximity to fruit croplands with intense pesticide applications (rural group –RG-) is associated with pregnancy complications and alterations in their newborn parameters compared to the urban population (urban group –UG-). A total of 776 pregnant women met the inclusion criteria. The percentage of threatened miscarriage was significantly higher in the RG than in the UG. The percentage of miscarriage, threat of premature labor, intrauterine fetal death, preterm premature rupture of membranes, and intrauterine growth retardation were similar in both groups. Newborn anthropometric parameters were corrected by sex and gestational age prior to statistical analysis. Length at birth and head circumference were lower in the RG than in the UG. Birth weight was similar in both groups. The percentage of head circumference less than the 5th percentile and the ponderal index were greater in the RG than in the UG. Our results suggest that proximity to pesticide applications may increase the risk of pregnancy complications and altered newborn parameters.

Cadmium (Cd), a toxic heavy metal, produces various forms of environmental contaminations and health problems in human. In this study, we aimed to examine the localization of several apoptotic markers in human placentas from pregnant women who were environmentally exposed to Cd. Twelve pregnant women participated in this analysis and they were divided into 2 groups according to their living areas: high-Cd (H-Cd) and low-Cd (L-Cd) groups. After delivery, the placentas were immediately harvested, and the placental width, length, and weight were measured. The placental Cd concentration was determined by using ICP-MS. The expression of three apoptotic markers, cleaved caspase-3, cleaved lamin A/C, and TUNEL, was examined in immunohistochemistry. In results, the placental Cd concentration in the H-Cd group was higher than that in the L-Cd group. In contrast, a significant decrease in the BW (birth weight):PW (placenta weight) ratio representing the placental nutrient transport function was found in the H-Cd group, and an inverse correlation between placental Cd concentration and BW:PW ratio was demonstrated. Additionally, significant elevations in the expression of cleaved caspase-3, cleaved lamin A/C proteins, and TUNEL were shown in the H-Cd placenta. Moreover, positive correlations were found between the placental Cd concentration and the expression of cleaved caspase-3 and TUNEL. Collectively, our findings suggest that the exposure of pregnant women to environmental Cd might induce Cd to be transferred to the body and then accumulated in the placenta, resulting in disturbance of the placental function and eventual apoptosis.
