Social [and health] relevance of psychotropic substances monitoring in air

Drug abuse assessment methods based on measuring illicit substances in waste waters are consolidated. The approach of ambient air monitoring looks questionable, nonetheless it can be explored if the variables determining the drug burdens are accounted for, or suitable co-contaminants are adopted to normalize concentrations to environmental and human contours. The general approach linking the airborne drug concentrations to consumption is presented and the case of cocaine is discussed according to measurements conducted in Italy. The cocaine/nicotine concentration ratio, identified as the most suitable tool, fitted well with anti-drug Police operations and people noticed for drug-related crimes, and with the abuse prevalence estimated in the cities investigated. According to that, the conversion factors of drug concentrations into prevalence estimates seem assessable, provided sufficient databases over space and time are collected. Further investigations are necessary to understand if airborne drugs cause adverse sanitary effects.

A limited number of studies have addressed environmental inequality, using various study designs and methodologies and often reaching contradictory results. Following a standardized multi-city data collection process within the European project EURO-HEALTHY, we conducted an ecological study to investigate the spatial association between nitrogen dioxide (NO2), as a surrogate for traffic related air pollution, and ten socioeconomic indicators at local administrative unit level in nine European Metropolitan Areas. We applied mixed models for the associations under investigation with random intercepts per Metropolitan Area, also accounting for the spatial correlation. The stronger associations were observed between NO2 levels and population density, population born outside the European Union (EU28), total crimes per 100,000 inhabitants and unemployment rate that displayed a highly statistically significant trend of increasing concentrations with increasing levels of the indicators. Specifically, the highest vs the lowest quartile of each indicator above was associated with 48.7% (95% confidence interval (CI): 42.9%, 54.8%), 30.9% (95%CI: 22.1%, 40.2%), 19.8% (95%CI: 13.4%, 26.6%) and 15.8% (95%CI: 9.9%, 22.1%) increase in NO2 respectively.The association with population density most probably reflects the higher volume in vehicular traffic, which is the main source of NO2 in urban areas. Higher pollution levels in areas with higher percentages of people born outside EU28, crime or unemployment rates indicate that worse air quality is typically encountered in deprived European urban areas. Policy makers should consider spatial environmental inequalities to better inform actions aiming to lower urban air pollution levels that will subsequently lead to improved quality of life, public health and health equity across the population.

Little is monitored on perceived neighborhood noise, quality of drinking water, air quality, rubbish, traffic, etc. at a continental scale. This study was aimed to examine the relationships of such neighborhood risks and mental health in adults and the very old in an international and population-based setting across Europe. Data were retrieved from the European Quality of Life Survey, 2007–2012 including demographics, living conditions, income and financial situation, housing and local environment, family, work, health, social participation and quality of social services. Adults aged 18 and above were included for statistical analysis (n = 79,270). Analysis included chi-square test, t test and logistic regression modeling. People who lived in town or city tended to indicate certain major problems for them such as noise (odds ratio (OR) 2.34, 95 % confidence interval (CI) 2.17–2.53, P < 0.001), air quality (OR 2.76, 95 % CI 2.54–3.00, P < 0.001), low quality of drinking water (OR 1.33, 95 % CI 1.23–1.43, P < 0.001), crime and/or violence (OR 2.92, 95 % CI 2.68–3.19, P < 0.001), rubbish (OR 3.68, 95 % CI 3.41–3.97, P < 0.001) and traffic congestion (OR 2.64, 95 % CI 2.45–2.85, P < 0.001). People who reported major problems on noise (OR 2.19, 95 % CI 1.96–2.45, P < 0.001), air quality (OR 2.11, 95 % CI 1.87–2.37, P < 0.001), low quality of drinking water (OR 2.40, 95 % CI 2.14–2.68, P < 0.001), crime and/or violence (OR 2.13, 95 % CI 1.88–2.41, P < 0.001), rubbish (OR 1.98, 95 % CI 1.77–2.11, P < 0.001) and traffic congestion (OR 1.50, 95 % CI 1.34–1.68, P < 0.001) were also classified as having depression. Perceived neighborhood conditions were associated with adult mental health across Europe. Future neighborhood monitoring research moving from the etiological to neighborhood management would be suggested.
