Simple methods for nutritional assessment in hemodialyzed patients

A mixed marasmus-like and kwashiorkor-like pattern of protein-calorie malnutrition was suggested by evaluations of the nutritional status of 18 hemodialysis patients. The somatic fat and protein compartments were assessed by means of anthropometric measurements. Assessments also were made of the visceral protein compartment and of cell-mediated immunity (skin tests). Diaries were used to assess the dietary intake of nutrients. Anthropometric indices, serum protein content (except immunoglobulins), and immune response were generally lower than in normal subjects. Although protein intake was normal, energy intake tended to be low. Protein intake was significantly correlated with the predialysis serum urea nitrogen. Additional experiments with intravenous short-term amino acid supplementation to improve energy intake showed only limited benefits.
