Clinical randomized trial of topical Quercus infectoria ethanolic extract for the treatment of chronic diabetic ulcers

Quercus infectoria has been traditionally used for wound treatment for many years in Thailand. This study aimed to clinically assess the efficiency of Quercus infectoria solution for the treatment of chronic diabetic ulcers in patients. Quercus infectoria solution was pharmaceutically formulated. A multicenter randomized controlled trial was performed in diabetic patients with ulcers (Wagner Ulcer Classification grade 1–3), who received standard wound treatment (normal saline solution) plus Quercus infectoria solution (study group; n = 26) or the standard solution alone (control group; n = 25) for three months. The primary outcome measure was complete wound healing. Percentage reduction in wound size was significantly greater in the study group than the control since week 4 (p < 0.05). During week 4, rapid wound healing was clearly observed in the study group with 60% wound size reduction, while only 30% was demonstrated in the control group. Approximately 90% reduction in wound size was revealed in the study group by week 8. In contrast, there were only 60% in the control group. At week 12, the reduction was 91.51% and 76% in the study and the control group, respectively. Moreover, the number of patients with complete wound closure in the study group (30.77%) was higher than the control (4%) since week 4 of the treatment. By week 12, 80.77% of the patients in the study group achieved complete wound healing while only 20% in the control group were completely healed. Quercus infectoria solution has been clinically proved to be of great potential as an alternative treatment for chronic ulcers in diabetes.
